Question: Had pain in my XXXXXXX with chronic headache for more than five years now. Did MRI on Cervical Spine which I have received just yesterday.

I need to know what this MRI can tell about my case, what is the diagnose and what is the right treatment.

Brief Answer:
Neck muscle spasm with low D3 levels.

Detailed Answer:
Hello,
Thanks for posting your query.
I have seen the attached reports. Your Vitamin D3 levels are low which can be responsible for a chronic muscle pain and fatigue. In the neck MRI, there is evidence of neck muscle spasm- may be related to posture. The chronic headache that you have can be related to the neck muscle spasm or due to other systemic causes like weak eye sight, anxiety, hypertension, electrolyte imbalance, migraine, etc.
For treatment, you need to start with vitamin D3 supplements and oral muscle relaxants after consulting your orthopedician. After the acute pain subsides, hot compress and regular physiotherapy exercises will help in reducing the spasm.
After starting the treatment in case the headaches are persistent then it is best to consult a neurophysician for proper evaluation and to rule out other causes of headache.
I hope this answers your query.
In case you have additional questions or doubts, you can forward them to me, and I shall be glad to help you out.

Your detailed answer and explanation are very much appreciated.
I'm sure the headache is because of the XXXXXXX (straightened cervical spine) which impact the vessels that supply the blood (Oxygen and nutrients) to the brain. The headache never comes alone. Rather, it starts with the pain in the XXXXXXX and then comes the headache.

I sit on my laptop more than 14 hours daily and I don't take breaks or weekends or annual vacations and this also confirm your diagnosis that this muscle spasm is due to posture. I now will have to get bigger monitor instead of the laptop screen so that I can have my XXXXXXX up during the work and I will get breaks as much as I can to reduce the muscle strain.

Would you please suggest:
1. Vitamin D dosage for my case along with Calcium supplement if needed (for calcium, I take Osteocare one per day after lunch, let me know if this is not suitable to my case)

2. Proper muscle relaxant (will I need Capsules, Cream, or both and what is the dosage). The pharmacist has provided me today with Eucal Herbal gel three times daily and Dimra capsules once per day. Please suggest better medicines if these are not suitable to my case or confirm if they are fine. I need the best that fits my problem.

3. What pain relied should I take, is panadol extra enough? should I take naproxen or alike?

4. When I was on Cerebromap once per day (Feb-March) I was really fine specially with depression and the bad mood, should I combine it with the above medications till I have the symptoms subsides? any undesirable effects for taking it for a month or two?

5. What is the expected duration for which I should be taking the medicines to have the acute pain subsides?

6. Do I need support pillows for sleeping? do I need special traction wedges?

Really appreciate answering my questions one by one.

Thanks again and looking forward your reply.

Brief Answer:
Details below.

Detailed Answer:
Hello.
Thanks for writing again.
1. You can vitamin D3- Tab. D3 Must 60.000 IU once a week with milk as a supplement. You can take osteocare in addition to it.
2. You need proper muscle relaxants- oral like thiocolchicoside. These need proper examination before prescription. You need to consult an orthopedician for that. Consulting a pharmacist will not help.
3. Panadol or naproxen are too strong right now. Take a mild pain killer like ibuprofen initially and get yourself regularly evaluated by an orthopedician physically every two weeks.
4. Do not self medicate yourself. Cerebromap should be taken only under a physician's supervision.
5. Acute pain usually subsides in 2-3 weeks.
6. Avoid using a pillow. Mild cervical traction at your physiotherapist's office can help.
Hope my answer is helpful.
