What Do My Blood Test Reports Indicate?

Question: Since a very long time, I have a tendency of frequent urination. In very short duration I need to go for urination again and again which often causes problem while travelling or while visiting somebody's place. Also, I need to think twice before drinking stomach full of water. Some of my blood reports are attached with my profile. Please suggest if this some medical condition or just a routine thing.
