What Causes Variable Results From Ovulation Predication Kit In Women With PCOD?

Question: Hello doctor(s),
We were planning baby since 1 and half year without success. I have attached all the reports for both of us and medications taken by my wife so far.

More details :
My wife has irregular periods and has been informed of mild PCOS. At times takes 2 months , or 3 months. Some times once the period starts it continuously bleeds till 1 month almost!!. We visited three hospitals, taken different kind of medications from duphaston, XXXXXXX kstat etc and never got the problem resolved. Everytime we go to a new doctor, they start the process from scratch again, it looks like we are in a loop and not coming out of it without any solution. Its not the doctors problem , it is we who changed doctors as there was no result and we were confused what to do as friends keep advising different hospitals. We were trying with one hospital
from the past 7 months now.

1) 3 months course - While she is on duphaston and related tablets(page 5 in pdf) those 3 months periods were as expected. But there was no success of conception.

2) Stopped above medication for next 2 months, but then we again observed the problem of irregular periods and continuous spotting to bleeding for days.

3) 2 months course - We were then advised to have XXXXXXX and related tables (page 6 in pdf). This was also fine w.r.t periods but no chances of conception. We were informed pregnancy will not result with the medications given here.

4) We tried Ovulation Prediction Kit which showed positive result on a monday, and again it showed positive on thursday!. We had intercourse on daily basis from sunday to wednesday, and then on friday.
We were hoping something positive this time as we used OPK test first time and was success. But we are back to square one - from the same friday she started spotting and bleeding , and this bleeding is going on for past 20 days :(

Questions :
1) Please observe all of our reports and let us know who has the problem and where it is and how we could go about addressing it?. Please do help us with good guidance as were stuck and confused how long to continue medicines, my wife is sick of having medicines ( she is having them almost daily since few years).
2) How do I know that we have ovulation as our OPK test gave success on monday and even on thursday, is it possible ?. Is there any scan possible to say ovulation actually happened as OPK test relay on surge in hormones, but after the surge whether actually released or not, will we know?.

I saw the reports attached.

Your reports seem to be normal.

1. Ovulation prediction kit is not reliable in case of PCOS . Better to go for Follicular growth scan to check growth of eggs.

2. Irregular periods are known to happen in women with PCOS But not prolonged bleeding.

3. She is likely to be having polyp in uterus due to which she is bleeding. Let her get hysteroscopy to check polyp and remove it if present.

Hi Doctor,

1) Please let us know how severity PCOS based on the reports and what are the medications you suggest ?.
2) Removing polyp should be a surgery I guess. Please advise if surgery is safe and no other impact or infections.
3) Do you suggest any good hospital in XXXXXXX for such problems. (in & around manikonda/tolichowki..)

1. PCOS is not severe. Moreover there is no permanent cure for PCOS.

2. Hysteroscopy surgery is a minor surgical procedure without more chances of infection.

3. I am not sure about XXXXXXX hospitals. You can go to any good gynecologist laparoscopic centre.

Hi Doctor,

1. How much would the Hysteroscopy procedure cost on an average?
2. I have a question on my reports as well -
My first semen analysis showed as Volume - 1 ML, count - 25 million, Active motile -35%, Sluggish - 40% and non motile - 25%. Normal morphology-85%.

And a week after, I had done test with another lab, now it showed Volume - 1.5ML, count - 72 million, Active motile - 50%, Sluggish - 10%, non motile-40%, Normal morphology - 85%.
Why is such a huge discrepancy especially in count and motility?. The way samples collected were in the same manner, no spills etc.. on both occasion,
3. Which report should I relay on?

Hysteroscopy costs about 10 to 20 thousand rupees .

Both reports of semen analysis are normal. You can rely on any one of them. The semen count always varies in a range and not restricted to a single count. As far as variation is in normal range, dont worry.

Hi,

1) Is D&C same as hysteroscopy ?
2) The variation in semen report is almost more than 200%, is it still fine?. How much variation is considered normal?. it changed from 25 million to 72 million, can it go down below 25 million too ?
