Question: I have lot of uncomfort in my throat. It doesn't pain while swallowing food but does while drinking water. Also, I have sore throat since last six months with hoarseness in voice. I don't smoke and my endoscopy of stomach was clear. Still I experience tenderness and swelling below Adam's apple with twitching and spasms on occasions.

Pain while swallowing liquids is suggestive of some infection.
The hoarseness of voice and tenderness at XXXXXXX Apple is usually due to laryngitis.
Please take an antibiotic like amoxicillin plus clavulunate 625 mg twice daily for a week. Get it prescribed by the local doctor/
Take a combination of diclofenac and serratiopeptidase twice daily for 7 days.
Take steam inhalation.
Take an antacid like pantoprazole 40 mg once daily.
Take a voice rest.
These all will help with laryngitis.

For the throat please send me the pictures of your throat so that I can suggest you better.
Also, tell is there any history of diabetes.

All the medicines I have prescribed will be available only on prescription.
Follow up with the required information.

Thanks for your answer doctor. I am attaching photographs of my throat. I have swelling under the jaw which resembles double chin. I suppose it might be lymph nodes swelling up. Also I have lot of belching and heart burn issues. The doctor I previously showed gave me rabiperazole and Domperidone for reducing acid reflux but it didn't help. I have done blood, urine, stool tests which are normal. I have had usg of liver which is normal. But this problem is persisting after all the treatment.

Please send me the pictures of your throat after opening mouth.
The double chin appearance can be due to lymph nodes enlargement or just fat.
Please get few investigations done like a thyroid function test,an endoscopy of larynx,ultrasound submandibular region and x-ray paranasal sinuses.
Your problem is not just due to one reason. As it is involving different areas, it should be explored properly.
Take the medicines as I prescribed earlier and follow the other advise too.
Avoid spicy foods and aerated drinks.
Drink plenty of water.

Thank you once again doctor. I am attaching pics of my throat with mouth open. I would also like to mention that I am having foul breath since many days now. I take good care of my dental hygiene and I am sure it is not related to dental hygiene. Could you also suggest a place in XXXXXXX where I should get it treated as I have already visited numerous doctors by now. Thanks

Thanks for the follow-up query.

The pain while drinking water is likely to be due to dental caries. Even the bad smell might be due to that. You have dental caries and dental plaques. You should visit a dentist.

The throat is inflamed. Please take the medicines as prescribed earlier. You will definitely be benefited.

Secondly get a laryngoscopy done and an ultrasound for sub mandibular area and X-ray paranasal sinuses done and send me the reports.

Along with other medicines also take tablet vitamin C once daily.
