What Causes Stinging Sensation In Penis At The Beginning Of Ejaculation?

Question: Hello,

I have a question. I have had prostititis in the past and occasionally get flare ups every now and again. I saw the doctor when i initially had it about 4 years ago and it was non bacterial prostatitis. Recently i had been ejaculating frequently however I had a period the last couple of weeks with no ejaculation. When I ejaculated after this there was an initial stinging sensation just as i was ejaculating not through the whole process but just as it started. I dont have any symptoms at all no burning on urination no pain nothing. Just this that i mentioned. Is this because of the fact that i was ejaculating regularly and then stopped for some time or could this be some symptom of a small flare up?

As I can see that your major concern is stinging sensation at the beginning of ejaculation.

I am suspecting primarily two conditions chronic prostitis that has exacerbated and urethritis.

In order to narrow down to a condition, I may need few more information
1) is there an fever, even mild intermittent fever? Chills?
2) Pain in groin area, pelvic area?
3) how is the stream of urine? Difficulty in voiding? Unsatisfying urination?

Stinging sensation after a break in usual frequency of ejaculation suggest inflammation of urethra. If there is no Fever, pain or burning while micturation, it is not much a concern and will subside on own.
However, as we are suspecting prostitis, we must investigate the condition as it can cause complication if not timely addressed.

I would suggest you few investigation
1) a complete blood count. Any inflammation if present can be cought by studying wbc s count.
2) A consultation to local urologist, as a per rectal digital Examination can be done to check if there is any swelling of prostrate.
3) If cost is no issue, I suggest getting a transrectal ultra sonography is better.
4) A complete urineanalyisis is suggestible.

The management and treatment depends upon the underlying cause, that can be found on investigation.

Hello,

Thanks for you response. There are no other symptoms. No burning when urinating stream is fine no difficultly voiding, no pain the perinium or pelvic area at all. I have had prostitis before and I am very familiar with the symptoms. The only sypmtom I have had in this case is stinging sensation right at the start of the ejaculation but not during the rest of the ejaculation itself. As for inflammation of the urethra I am not sure why I would get this as I do not have any other specific symptoms. Please advise. thanks

Your information clearly negates the possibility of active prostitis.
Although your symptom is explainable by presence of a mild urethral inflammation. The irritation and stretching of urethral mucosa during ejaculation must have been a reason for the stinging sensation. The cause of inflammation could be a minor infection of the urethra, whose source could be any. As there is no burning during urination, no pain and fever, there happens no concern or need for any active intervention. Therefore, waiting and observing the condition for next couple of days is wise. If the pain persist or increases, consult a urologist urgently.

Although, it would still strongly be suggestible to consult a local urologist as a clinical picture of chronic Prostitis may not necessarily be a typical one. A prostate examination is still indicated. Also a complete blood counts and urine anlaysis of the first urine stream, that can help us knowing if there is any site of infection in urethra.
