What Causes Stabbing Pain Under Right Breast?

default
Posted on Mon, 12 Oct 2015
Question: I am sorry to bother with my question. I get a pain on my. Left side right under my breast. The pain comes and goes. Sometimes I will get it once a day other times just starts and then it will subside. It feels kinda like a stabbing pain. When get that I feel my heart thump harder. Sometimes when it happens I lay down on my left side and just take deep breathes to calm. And I can feel my heart thumping stronger? Harder. That I can literally feel the beats. I haven't gone to get it checked. I have to wait to get insurance. The other day the pain was at it again but this timet he pain was more. That even my deep breaths wasn't helping. I do have gerd but I can't say this is from heartburn. I know its not that. Should I schedule myself with a cardiologist specifically? And what test should I ask for? Now I did get my breasts checked I wanted to make sure it wasn't from the lump in my breast and it DEFINATELY is not that. Because when I get the stabbing pain, I feel my heart I can feel it on that side. I hope I am not being just paranoid. But I would like to know what you would recommend I do. Now when I have had this happen, by the time I would get to the dr it would be gone. I can't recall how long ago, but I did get a echo of my heart done maybe like 6-7 years ago. The echo came back fine, but I was never asked to wear a holter monitor. Please give me your advice. Thank you and have a good day.

Detailed Answer:
Hello!

Welcome and thank you for asking on HCM!

I understand your concern, and would like to explain that, based on your pain characteristics (stabbing pain modulated by posture and deep breaths), it doesn’t seem to be cardiac related.

A cardiac ischemic pain isn’t like this. So relax! You don’t need to think first of a cardiologist as your pain is most probable to be of extra cardiac origin.

I recommend you to consult with an internist, as a comprehensive differential diagnosis should be done.

Several causes like a musculoskeletal disorder, a backbone degenerative disorder, a pleural affection (inflammation or adherence), esophageal spasms, pulmonary micro embolisms, or even shingles (herpes zoster), etc should be sought.

A careful physical exam, a resting ECG, chest X ray study, several lab tests, like a complete blood count, PCR, D dimer, LDH, AST & ALT, etc. would be helpful for the differential diagnosis.

If suspicions are raised, then chest angio CT, cardiac ultrasound (for measuring pulmonary artery pressure and investigating right heart chambers), or lower limbs Doppler ultrasound (for investigating possible vein disorders as a source of pulmonary emboli), etc should be done.

You need to discuss with your attending doctor about the above mentioned issues.

Hope to have been helpful to you!

Feel free to ask me whenever you need! Greetings! Dr. Iliri
