What Causes Severe Facial And Ear Pain With Headache, Tinnitus And Blurred Vision?

Question: I started to have pain yesterday on the right side of my face/jaw which made it hard to move my jaw. It has now gotten so severe I am in tears, and it has extended to my head and neck and the pain is causing severe head pain (headache) facial/jaw and ear pain into my neck. i hear noises in my right ear, like cracking and ringing and some blurred vision. I feel nausea like I'm going to vomit. My right side is also numb and tingling and shooting pains all through my face and head and neck and into my ear. I feel horrible and the pain in unbearable now and I can't seem to find relief.

Plus my blood pressure is normally 122/77 and today it's at 166/99

Yes there is swelling and pain when i move my jaw. No issues with my teeth, severe pain in head, ear, jaw, face and numbness, tingling. Vision is blurred but not always. Pain is throbbing and shooting all thru head and face and jaw and ear. Loud ringing in ear. Yes i have had migraines before. My face is cold too, and it hurts if i try to eat or open my mouth at all.

Hello, Welcome back to HCM.

It is a bit unusual for migraine but basilar migraine may have such features.

You should take Tab.GRENIL(a combination of PARACETAMOL 500mg and DOMPERIDONE 10mg) 1 tab Twice daily after food for next 3-4 days at present.

Take rest, avoid light stimulation.

In future take regular timely diet, proper sleep, no fasting, avoid chocolate and excessive coffee.

Consult ER if not improving. Hope you found the answer helpful.

No the tablet mentioned above is not narcotics.
You can take any pain killers like Naproxen, Aceclofenac or paracetamol too.

Sorry to bug you again, but i saw the doctor yesterday and he told me to contact an oral surgeon, that this isnt something he can help me with. However, he then tokd me to watch out for a rash that is sounds like shingles to him from the symptoms i described, as i described to you. Well i got no rash today, going on three days of severe discomfort and pain. He told me to take two aleve PM and rest. And if i woke up with a rash just come in to see him today for a anti-viral shot. Thats it!! Im so upset and hurting so bad i want to shoot my head off. I wouldnt do that of course, but thats how bad it hurts. I just want out of this misery doctor. What should i do? He also said the ER cant help me. I found an oral surgeon and he tells me i need to be referred to him. All this waiting while i sit and cry in pure afony. help!!

Opps I meant pure agony!! My PC was of no help yesterday and I'm still suffering
and I don't know what to do. What should I do?

Hello,
If your oral examination is normal and headache is not the prominent complains then possibility of Varicella can be kept.
In some cases the involved segment present with severe pain followed by skin eruption.
Anti viral to be started after evidence of skin lesions in distribution of nerve.
Pain can be reduced by taking pain killers, tramadol (opioid )along with pain modulator like carbamazepine, gabapentin, pregabalin or amitryptiline.
Get consultation and visit to a neurologist for prescription.
In such a situation I prefer to give pregabalin 75 mg and amitryptiline 10 mg with tramadol in my patients.
Do get back to me for further queries.

I will!! Thank you for all your help and advice. My PC said that the rash should surface and quickly by today. He said with the onset of all this pain and the symptoms I described, he said within 48 hours the shingles virus would surface.
He said to look out for it today and since the pain started around 3am on the Dec. 8 I would be seeing by today and still nothing, not even a bump or spot(s).
The aleve is not helping and I'm taking two every four hours now not six.
So you think I should see a neurologist verses a oral surgeon? This pain is severe and all through my head and ear is killing me and into my neck. It feels like it is spreading further to all over my head and face and forehead and back of my head and down my neck to the top of my shoulder. My face is extrememly swollen too and still numb.
It takes time to set up all these appointments and time isn't something I can handle, and I can't believe the ER can't help me at this point. I mean I think I need a MRI or something to see what is going on in there.
Nothing is helping me at all.

I reviewed your photo and could make that lower face jaw region has some swelling.
Is there any toothache, eruption of wisdom tooth or any caries of teeth on the same right side.
Other pain as discussed earlier may be neuralgic pain due to irritation of nerves supplying.
Is your headache more severe or your facial pain?Facial pain is more in Upper face or lower face?

For treatment I would start you on pregabalin 75mg along with amitryptiline 10mg later on with tramadol as and when required.

Get dentist appointment if you have any teeth issues else prefer a neurologist or physician for evaluation and investigation.
