What Causes Severe Cramps, Abdominal Swelling And Black Bowel Movements?

Question: My history: My name is XXXXXXX I am a 50 year old woman who had a partial hysterectomy (I have my ovaries) at age 26 due to severe endometriosis. I had bleeding ulcers and hospitalized in January 2012 and received 5 pints of blood. Later this year, I finally was treatment for Lyme exposure. I had various stage symptoms undiagnosed for 10 years and after the symptoms were effecting my work (brain fog, severe joint pain, muscle aches, head aches and racing heart, etc...) I finally went to a Lyme expert. I was placed on 6 week round of antibiotics then a proven herbal protocol of Samento, Banderol, Burbur, Serrapeptase and Pinella. I was actually seeing an immediate improvement and most symptoms went away. Best I felt in a long time.
Finally a doctor who listened. I also took a daily shot of Semorelin Acetate 15 mg with GHRP (2) Theanine 75mg due to life long sleep issues and many failed attempts of other medications, sleep studies etc... I actually sleep for the first time in over 20 years. The U.S. Marshalls take it for sleep issues. I take prescription drugs Topiramate, Pantoprazole, Nucynta (when the pain was get bad). I also have MTHFR gene mutation so I don't absorb vitamin B well and take homocysteX supplement for that. About 30 days into the 2 - 6 month treatment, my stomach started hurting and then I noticed black stool again which meant my ulcers were bleeding. My Dr. (who I love) stopped all of the herbal protocol until bleeding stopped which I was told it had as my hemoglobin was going back up on the last test. However, every time I took my nightly shot, I received pain immediate nausea then pain in my abdomen. Two days ago I decided to stop the nightly shots as well even thought Semorelin Acetate is suppose to be what your body naturally produces. Today, the dull pain is still there near the navel (right side). Remembering back it has been for about a month. I eat not becasue I am hungry but becasue if I don't I get nauseated but then feel extremely bloated and stomach hurts afterwards. lose lose situation. It is almost like when I had my periods. the cramping, bloated feeling and pain. I just would not know becasue I have not had one since I was 26. I have abdominal swelling, no fever, I have bowel moments that are still black but the iron supplement I take could be causing that. I do have severe gas feeling and cramps - last night was severe but better this morning. I researched some on line and I see everything from appendicitis, ovarian cancer, irritable bowel, hernia? I am had 4 or five colonoscopy and endoscope procedures over the years which never found anything. I have regular physicals but I guess anything is possible. My Mom had cervical cancer and I think ovarian. UGH! It is Sunday and of course my work day tomorrow is the day from hell - 7:00am to 9:00 pm as I work in Intergovernmental Affairs for the City and it's budget time. What are my options? Obviously if serious and life threatening, I will go to an emergency room but these days hard to tell. I lost my late husband three years ago suddenly and just remarried and now this. He is really worried. If I had appendicitis would it not have ruptured by now? If I have ovarian cancer isn't there a blood test that can be complete ASAP to rule this out? It will take days or even a week to schedule an MRI or endoscopic test. I would appreciate a detailed real response from any doctor who has experience in this field.

Thank you.

Brief Answer:
Possible bleeding duodenal ulceration.

Detailed Answer:
Hello, Madam.

I worked through your query in detail and understand your concerns.

The possibility of duodenal ulceration is very high.

Bleeding ulcers can bring almost all the symptoms which you mentioned.

An upper GIT Endoscopy is mandatory in your case at the moment.

At the same time, I would like to Stress the importance of MRI abdomen to assess the state of ovaries and liver considering the due importance to the side effects of. all the drugs you used so far.

Further management if any is based on the test results.

Meanwhile, I advice you to discontinue Nucynta and take Pantopprazole twice a day.

Appendicitis, hernia, IBS are unlikely in the current context.

Post your further queries if any.
Thank you
