What Causes Severe Chest Pain And Breathlessness After Suffering Physical And Mental Trauma?

Question:

This is gonna be a long post. I have been under physical and mental trauma for last 7-8 months. It all started as episodes of breathlessness in the night. 2-3 nights continuously in April. I had consulted a MD Chest and my reports came normal except my IgE level was inflated to 637+ I was put on Allegra. Then After 15-20 days I developed the problem of rapid heartbeats and palpitations along with episodes of breathlessness and chest discomfort and I got it checked by MD Cardiologist. ECG and Echo 2D was done and all came back normal. Only diagnosis wasMVP Grade 1 in Echo 2D. These palpations and chest congestion problem was occurring on and off for 2-3 months.

In September, Again there has been episodes of breathlessness, a feeling of shallow breathing when I go to deep sleep and I usually got up as a jolt in the chest because of Shallow breathing and then followed by rapid heartbeats for 2-3 mins after that I came back to normal. This happened to continuously 2-3 days. I again went to GP this time another MD chest and he denied the possibility of Sleep Apnea. I went to see another Cardiologist and he recommended a holter monitoring . The tests were normal according to him ( Though the slowest heartbeat was 57 and the highest was 147)

Then that doctor put me on Inderal 40 mg ( My BP comes usually normal, sometimes it comes 130/85) and advised me to check my lipid profile, sugar and thyroid. My sugar and thyroid came normal. But My lipid profile came high ( Total Cholesterol 240 , LDL-136 , HDL-50 and triglyceride 106) along with an inflated level of homosysetine of 45 and Lipoprotein(A) was around 50. I was told LP(A) is hereditary and it cannot reduced substantially. I was told to reduce my cholesterol level. Treadmill test was also done and it was normal as well.

Now I'm taking Inderal 40 mg ( My rapid heartbeats are normal, usually occurs when I exert a bit ) and Rosuson 10 mg for cholesterol and Homocheck tablets.

The problem is now it has started occurring again, the chest discomfort usually, a fisting pain on my chest ( I've heard of unstable angina though I'm finding it hard to believe in my case my almost all reports came normal ) and a feeling of nausea and chest stiffness when I usually exert ( brisk walk on treadmill but I still manage to brisk walk for 20 mins daily on it but meanwhile I feel thirsty nauseatic and feel a discomfort on chest ). And for last 2 days the episodic nights have started reoccurring in which whenever I go into deep sleep I have to get up as if there is a jolt on my heart followed by palpitations for 2-3 mins till I get normal. I have also been suffering from heartburn ( acidity ) and feeling of being full after meal.

You can very well judge my frustration by the length of my post. I have been to 4-5 cardiac specialists and I came back as nothing serious but these episodes of frequent discomfort in daytime and episodic nights are making so sick and physically and mentally distressed.

I want to know all these symptoms show any heart attack or any cardio vascular diseases in waiting ? I feel scared to death. Help me out.

Brief Answer:
your symptoms are not cardiac

Detailed Answer:
hello,
I am Dr.Rajesh Teli,MD.
I have gone through your query. Thanks for using HCM.
I understand your frustration.I would like to know some basic details of your personal history.
1.How old are you ?
2.What work do you do ?
3.Do you smoke or consume alcohol or take any other medicines ?
4.Are you a female ?If so please inform about your menstrual history.
5.Do you have family history of Diabetes,Heart disease or Asthma ?

Looking at your frequent spells at night it is likely that you are having significant Acid reflux or it may be hiatus hernia promoting acid reflux.I suggest you to get upper GI endoscopy to confirm or rule out this condition.
Unstable angina is less likely cause your TMT was normal.I request you to attach TMT tracings for my personal interpretation.
meanwhile i suggest you following measures
1.Avoid heavy meals and take small frequent meals.
2.stop tobacco, smoking or alcohol if at you consume
3.Maintain 3 hours gap between meals and sleep.
4.Avoid spicy,fatty meals.
5.You may start taking Tab.Rabeprazole and Domperidone combination once a day before breakfast along with Inderal and Rosuvastatin.
6.you may take anxiolytic like Tab.Alprazolam 0.25 mg at bed time.

Brief Answer:
you need not worry about heart

Detailed Answer:
Hello Mr XXXXXXX
I have gone through your TMT report. It is quite satisfactory and you need not worry about heart.Your symptoms are not due to heart disease.
As I suggested earlier you should get your Vit.B12 and Vit.D3 level checked.
Start with rabeprazole with domperidone along with aplrazolam at bed time( as suggested earlier)
If not improving, you should consult a gastroenterologist, for endoscopy.
Please don't get panic and relax everything will be fine,
My best wishes
