Question: I woke up in severe belly pain 2 nights ago, it only lasts 30 seconds, but is unbareable. Then I was fine for hours. It happened 2 other times. Then had diarrhea. I was fine again. Now today I have very foul smelling gas. Any thoughts of what is going on?

Brief Answer:
Intestinal infection with occasional sub-acute obstruction.

Detailed Answer:
Hi.
Thanks for your query.
I have read your history and understood the concerns.

The most probably cause of such an occurrence of sudden severe pain lasting for 30 seconds followed by diarrhea and then foul smelling gas can be due to intestinal infection.
The sudden temporary pain can come due to intestines trying to push the infected material and entrapment of gas causes such pain.

I would advise you the following:
Send stool for tests, blood tests.
X-ray of the abdomen in standing position and ultrasonography.
A course of an antibiotic, probiotic as prescribed by your Doctor.

I hope this answer helps you to get an early diagnosis and proper treatment.

If you are referring to a stomach bug, I know it wasn't that. This has happened before, although I feel like it is my intestines. Sometimes I feel something on my right side. Wgen this has happened in the past the gas is burning but that didn't happen this time

Yes, it is related to intestines as you have mentioned.
Foul smelling gas is produced by the gas producing bacteria.

How do you know it's intestines and not appendix or gal bladder issues

Brief Answer:
As explained below.

Detailed Answer:
The answers provided are depending upon the history you have provided. You have spasmodic pain followed by diarrhea and found gases is suggestive of intestinal pathology. Such things do not occur in gall bladder issues. Appendicitis may give similar problems but the pain will be more on right side and more persistent than you have.

It's happened a handful of times in the past year. I was definitely blared the day before it happened. The sudden severe pain came on a few times then I had loose stool hours after. I do not have diabetes. I have goiter hypothyroid. A year ago I had a ultrasound done because I had a consistent pain in my right side but didn't find anything, not sure if it's related to my current issue

Brief Answer:
As detailed below.

Detailed Answer:
Thanks for your feedback.
With the history you have added on I think mere ultrasound might not have been sufficient to clarify the recurrent problem.
I hope your thyroid function tests area normal.
I would advise you to have proper clinical examination and investigations to get a proper diagnosis. Since there are recurrent problems it is better to get proper diagnostic procedures:
Tests of blood, stool and urine.
Contrast enhanced CT scan of the abdomen may reveal the problems as Ultrasound is not capable of finding many of the diseases because of its limitations.
It can detect appendicitis, colitis or any other inflammatory disease diverticulitis, sub-acute intestinal obstructions and so on.
If required further tests like colonoscopy and biopsy may be needed.
Have proper clinical evaluation by a Gastroenterologist or Surgical Gastro/General Surgeon.
Keep thyroid functions normal by proper medication.

I hope this answer helps you further, please feel free to ask for further relevant queries if you feel that there is a gap of communication.
