What Causes Right Sided Abdominal Pain?

Question: Hello, Dr. Rynne. I've had R side abdominal pain, on/about the GB area x 2 plus months. GB removed 2 years ago. CT/ Ultrasound, no findings causing pain( I had a liver and kidney cyst). Labs do not indicate any abdominal changes / issues. Any ideas?

Your test results do show a clean chit, but if you do not mind I would like to ask you a few questions in order to try and find the cause for your pain. Please take time out to try and answer them so I can help you.

1. How would you rate your daily physical activity (meaning how active are you) on a scale of 0 to 5 (0 being completely immobile and 5 being hyperactive)?

2. How many litres of water do you drink per day?

3. How often do you pass your bowels (e.g. once daily, once every alternate day, etc.)?

4. When you do pass your bowels, how much stool (approximately do you think you pass), just guessing visually (in grams)?

5. Is your diet rich in fiber? Could you give me a general description of what your usual diet consists of?

6. Your weight and height please?

7. Do you experience abdominal bloating? If yes, how often?

8. How many meals do you have per day? And how far apart (in hours) are they?

Crossfit/ x2 years, stopped recently due to abdominal pain and extreme tiredness. I'm 5'5/ 133, no weight gain or loss. Yes, abdominal bloating. Bowel movements x 3-4 weekly. I eat pretty clean with an occasional cheat day. Prolly not enough fiber. I've had mediprep and had a total of 9 movements. I feel feverish but undocumented. I am achy , flu-like. Today, I got nervous and the pain in the abdomen was noted on the upper left as well. Labs show blood in urine x2 months, recently tested positive for bladder infection. Abdominal bloating ... Yes. In my first writing I failed to mention that I started a new medicine , XXXXXXX lactone? Spelling, for derm reasons. No problems prior to XXXXXXX but dr doesn't think problems relate to rx. I am desperately tired with continued r side pain. Going to GI and GYNO for well care. Any ideas or questions I should ask... Thank you again for your help! ** sore throat today, headache, persist x2 weeks.I eat approx 5 - 6 small meals per day... Again lots of protein, chicken , salmon. I feel infected but labs are normal. During this journey I've had one lab return with a slightly elevated white count. As of 4/18, count returned to normal. All other labs are normal. Well, estrogen spiked from 156 to 466 for a couple of weeks but is now normal.

Thank you for the details provided, here are my inputs:

+Your irregular bowel pattern and the CT scan reports do point towards possible gradually worsening constipation, and I would recommend a colonoscopy for this reason (in order to rule out any obstruction)

+Please increase your fibre intake

+Also initiate probiotics as part of your daily diet (either natural or prepared ones from grocery or medical stores)

About questions that you can ask your doctor, here are a few:
1. For gastro:

+Discuss ruling out conditions like irritable bowel syndrome: Which can present in the form or diarrhoea or constipation

+Discuss options to initiate treatment of constipation (e.g. daily laxative)

+Discuss diet plan to increase fiber intake

+

2. For gynecologist:

+As I see you have had a fluctuation in your hormone levels without any apparent reason, and your age also seems about right, please discuss the possibilities of your presentation being related to peri- or pre-menopause, which very often presents with gastric symptoms.

Symptoms like headaches, gastric symptoms, abdominal bloating, exhaustion, lethargy, can all be associated with peri- or pre-menopause. So this is an important question and detail to be discussed.

Before visiting your gynaecologist, make sure you take a detailed history of your menstrual cycle along with you (e.g. any changes in duration of period, frequency, amount of blood discharge, any dryness or pain experienced recently during sexual contact, etc.)
