Brief Answer:
Antibiotics and local care as explained.

Detailed Answer:
Hi.

This type of a sore in peri-anal area is usually due to an infection of the intestinal tract.

I would suggest in such a patient the following and always helps ::
*a course of an antibiotic like cefixime and metronidazole for at least 5 days. You will need a prescription for these medicines. You local Doctor will help you to have a clinical examination and will give you'' Prescription''.

*Also have multivitamins and probiotics.
* No local applications of any ointment. The only solution that can be applied is Liquid Povidone Iodine (the area becomes dry soon). This will help to kill the bacteria at the local level. The oral antibiotics will help to cure your intestinal infection and local infection too.
* Keep the area clean and dry.
* you can have a ''Sitz'' bath- meaning, you have to sit in a tub with warm water before and after you pass motion. You have to clean the area with cotton soaked in the water in this squatting position to remove the traces of feces which are not easily cleaned otherwise.

You could have posted the photograph.

This type of care will heal the sore in most of the cases unless there is local skin condition like eczema or so. In this case you may have to take an opinion of the Dermatologist ( if the above therapy does not help you) to get a correct diagnosis.
