What Causes Persistent High Temperature,fever,sweating And Body Chills?

Question: Last week I sent a query in regard to my friend which was answered by both a Cardiologist and an Endocrinologist, however, her health is no better.

My friend has been having persistent high temperatures, fevers, sweats, chills for the last six months and during this time her temperature has rarely been within normal range. She has seen an Internal Specialist, Neurologist, Lung Specialist, Cardiologist and has had an MRI, Pet Scan and other tests and blood tests. Since I asked the question last week where you indicated it may have to do with her adrenal glands, she has received the results back from the 24 hour urine test; along with associated blood test that's done in the morning (ACTH Test) and whilst she does not have copies of these results she was told the results were normal and she most likely does not have Pheochromocytoma.

She continues to experience the aforementioned symptoms; along with episodes of light-headedness that last between 10-20 seconds, episodes of dizziness which are generally worse upon standing and are eased a little by laying down with her eyes closed. She also experiences an irresistible urge to stretch out her legs as far as possible, like she can't relax them and feel like her legs are having a seizure - this mainly happens at night. Her most recent temperature reading (using an ear thermometer) was 39.8. She has not been able to work for the past couple of weeks.

What are your thoughts as to what is causing her unrelenting symptoms and are there any tests you could recommend?

Thank you for posting your query on HCM.

Well looking into your problem I would like to tell you that Since she has undergone a bettery of tests which you say is normal the we need to see for rarer conditions .

First there is a very high suspicion of autoimmune disease like lupus presenting like this , so I will recommend ANA , ENA Profile , XXXXXXX testing to be done .

Next on cards will be lymphoma , it can present with fever like this for which CECT chest and abdomen should be done and on physical examination to find if any lymph node is enlarged .

Tuberculosis is the greatest mimic for such problems and mantoux test , TB gold quantiferon tests should be done for the same

In such symptoms which dont fit in a category extensive testing is required to get a clue . If you have any previous abnormal test report please send it to me.

Thank you for replying so promptly. My friend said she has had an ANA test which was normal; as was the CT Scan to check for lymphoma. She has an appointment with a Neurologist today so will ask him about lupus etc.

Just ask about possibilty of tuberculosis also , if it has been ruled out or not. ENA and XXXXXXX should also be done to see for vasculitis or other connective tissue disorders .
