default
Posted on Fri, 7 Oct 2016
Question: Good morning. I am a 51 year old female. For several months I have experienced muscle and joint pain througout my body. In general I feel fine when I am up and moving, but sitting or lying down for any period of time causes my muscles to become stiff and sore. Do you have any ideas about a cause or what I might do foe relief?

Brief Answer:
rheumatic condition, functional pain disorder, depression

Detailed Answer:
dear,
welcome to Hcm
After going through your complaints, I would like to consider possibilities of autoimmune, rheumatic conditions, general infection and functional pain disorder.
For which I would like to know
1) Do you experience more pain on waking up in mornings than the one during evenings?
2) Do you feel that you are getting tired easily than before?
3) Does your joints feel tender on putting pressure?
4) Is there an associated medical issue such as Thyroid dysfunction, hypertension, etc?

Investigations suggested-
1) ESR , Complete Hemogram, vitamin B12 levels.

Information regarding these questions and the investigations shall help me to narrow in on a more specific cause.
Meanwhile, I would suggest you to use over-the-counter pain medications available in the market.
I would suggest-
tab (Ibuprofen+paracetamol) two times and if required three times a day.
tab ranitidine 150mg once a morning
take these for three days.

Do not worry as there appears no indication suggestive of an emergency or grave medical condition.
Kindly revert back with the information.

Regards
Dr Pankaj

Thank you for responding to my questions. To answer your questions:
1. I usually experience more pain in the evenings. I have stiff joints and sore muscles if I sit too long during the day but it works itself out when I get up and move around. However, in the evenings when we sit down for supper the stiffness in my joints and the muscle aches are awful. The aching in the morning is much less and gets better with movement.

2. I do feel more tired than I had in the spring/early summer months.

3. I do have tenderness in my joints.

4. I do not have any associated medical issue such as Thyroid dysfunction or hypertension.

Brief Answer:
Non specific inflammatory condition

Detailed Answer:
Thank you for the reply.

The nature of the pain is suggestive of a rheumatic cause. A possible non specific inflammation of tendons and joints.
Easy fatigue, tenderness, stiffness, etc are characteristic of such conditions.
An associated back pain(if present) would further support such a possibility.
Stress, infections, seasonal changes, etc does tend to flare the pain.

I am suggesting few investigation
- ESR, complete blood Hemogram, XRay of the joints involved.

These can help us to lead and strength our diagnosis.

You can continue the medication as advised before.

Again, there is no reason to worry as there is nothing suggestive of serious medical condition. However, do consider to undergo the investigations soon.

You can revert back here with further information and queries.

Regards.
