What Causes Occasional Chest Pain, Muscle Pulling And Pain In Left Arm?

Question: Hi,

I am a 38 year old male.
I am experiencing occasional pressure in my head with occasional chest pain and slight shortness of breath now a days. My triglycerides are at 280 and my AntiCCP is positive. I also have lipoma's near the chest and have had them for sometime. Sometimes I face nerves/muscles being pulled on my left arm( arm pit and hands, arms). What would be the problem here?
I am able to walk long distances without issues. But in general I have this uncomfortable feeling throughout the day. My sleep cycle is normal.

I share your concern. Based on your description, I am concerned the tightness in chest with shortness of breath could be related to a heart problem. Since your triglycerides are also high, there are chances of narrowing of blood vessels leading to a heart condition. I would like to know about your weight and complete medical history like diabetes or other metabolic disease. Whether the symptoms develop on exertion or at rest.

AntiCCP postivity is suggestive of rheumatoid arthritis or related connective tissue disease

Muscle pulling and pain in left arm is again favouring the heart problem most probably.

Lipoma is usually not symptomatic. Therefore I don't think they are related.

In this context, I suggest you see a doctor at the earliest. You should consult a cardiologist for recording blood pressure and undergo investigation like EKG, ECHO and complete hemogram, liver function test or serum cardiac markers if need be

Thanks for the reply.

Some time in Nov, I went through stressful times as my mother was diagnosed with breast cancer. During that time, I went through lot of anxiety. On one day, I went though something where I felt pressure from my lower back passing through to the head and then releasing from head back down. This occurred for almost 15 minutes and would come back when I walk for sometime.
After this, I have been having one or the other issue over the past six months. I had severy acidity issues and took advice from a gastroenterologist. Took Nexpro 20 for three weeks and that reduced.
The physician whom I initially consulted multiple times, said that this was an anxiety attack and there was nothing wrong with me.
But days have passed and at present in the past three weeks I am having the symptoms I described. Would it be possible that I am having the chest congestion due to hyperacidity? When I was on Nexpro RD, I did not have such issues and also my head pressure was also away.

Looking at your detailed description this could be anxiety neurosis or acid reflux disease but in my opinion you should rule out the heart problem as earliest possible.

Hyperacidity may cause chest congestion at times
So i would advise you to continue nexpro 20 with antacids
Avoid fatty fried spicy and junk food
no alcohol or smoking
Have lots of fluids and fresh fruits
Yoga and meditation will definitely help you

Hi Doctor,

I had a few tests done a month back. This included a treadmill test and blood/urine tests which included Haemogram, Lipid Profile and Liver Function tests. Can you please look at it let me know if anything further would be required to conclude on hear disease?

Your ECG is suggestive of sinus rhythm with rapid heart rate which can be related to anxiety

Your all reports are within normal limits so you need not to worry
Here I would advise you for thyroid function test and an Echocardiography

Hi Doctor,

I have uploaded more reports that I got done on 19th March with Thyrocare.

This includes the Thyroid test as well.

I will get an echo done shortly.

Can you please let me know your observations on the Thyrocare reports?

Almost all your reports are within the normal range except anti CCP antibodies, low testosterone and high triglycerides
In my opinion your reports are not supporting your problem
I would advise you to consult a psychiatrist for counselling and management of anxiety through various relaxation exercises

High triglycerides can be managed at your age by dietary restrictions and life style modifications
