What Causes Lower Pelvic Pain Radiating To Back Along With Irregular Periods?

Question: my right ovary is causing me constant pain. on going for a couple months. pain goes through to my back and on occassion down the inner thigh of my right leg. ct scan showed nothing wrong with kidneys, spine, gallbladder, appendix, etc. no pelvic fluid. blood work all normal except for low platelets. normal pap. had ultrasound and that was supposedly normal although my right ovary was hiding and couldnt be seen clearly, it appeared cloudy. I have a loss of appetite, and fatigue as well. also irregular periods, spotting between periods and pain during intercouse. ive had my tubes tied and 2 c sections. pain is constant although the severity of it varies. sometimes its really bad and others it is manageable. it seems like no one can tell me what the cause is. what do i need to do to figure out why this is happening? what could these symptoms be?

Thanks for the query to HCM,
The typical finding of right ovary not been seen on imaging studies and typical irregular inter menstrual spotting with pain or dysmenorrhoea indicates toward a possibility of ENDOMETRIOSIS.
The pain is typically associated with menses and spotting and there can be adhesions between adnexa and right ovary and hence not being visualised.
The suggestion would be to rule out the same with diagnostic laparoscopy , which is the gold standard for diagnosis. As all investigations have been normal including the kidney function tests ruling out UTI, hence endometriosis is the likely possibility. Pain during per vaginal examination and intercourse is again typical for endometriotic lesions in the pelvis.
