What Causes High Serum Creatinine Level And Weakness?

Question: Sir, My father-in-law(78yr) was having hypertension for 7 years and it was controlled by medicine and was taking ecosprin av-75 regularly.Suddenly he fell down and vomitted 30 days before and admitted to hospital.After all test he was declared to have arhythmic heart beat.After 7 days he was discharged with medicines- cordarone100, fruselac,czpam,predmet 4(1 daily) ,gastractive,pantodac 40 and deryphyline, ambron syrup and inhalers tiova, seroflo and levolin for cough(doctor said it copd, but it was quite habitual cough). After 12 days he vomitted again and doctor then gave zofer 4 thrice a day for 7 days and systel for 7 days.At that time creatinine was 1.98(range:.6-1.4) ,doctor told to continue all the medicines for 1 month. Since a week he is feeling very week and not passing stool after giving 10 ml laxose bedtime a day. I checked his sodium:125(135-155),pottasium:4(3.6-4) and chloride:90(98-108). His bp is now:130/65 without any pressure medicine. What is the reason of weekness? He is having norrmal low-salt diet. Does any medicine creating any problem to him? What about bp? What is significance of creatinine 1.98? Is it a problem and what is the remedy?How can the weekness be overcome? He does form acidity and gas very easily and cant have fruits.How can we treat him? Now he has no cough, only fatigue. How can we regularise bowel movement? Pls. help.We are at dark. Kindly help.

Brief Answer: Can be many reasons Detailed Answer: Hi Thanks for posting your query on HealthcareMagic. Your father in law's serum creatinine is higher than normal. It may be suggestive of kidney issues. It is possible that during the phase of arrhythmia he may have suffered damage to kidneys (particularly glomerular region of nephrons) called as acute renal failure. It may be one cause of weakness. I would suggest that he be tested for GFR (glomerular filtration rate). Another reason for weakness could be amiodarone induced hypothyroidism. Thyroid profile (a blood test) may be done to rule it out. Czpam is a benzodiapine and may cause drowsiness. Regarding bloating, try giving him cleaner water from reverse osmosis technique. His sodium levels are also low and may be contributing. Blood pressure is in normal range. We need to find the cause then it can be according. To regularize bowel movements he can try rectal suppositories. Hope it helps. Let me know if I can assist you further. Dr Vaishalee

Dear Doctor, His thyroid is within normal range. BP is 125/70. sodium is increasing. Presently he is feeling better. He is being given ors (in 200ml water) 2/3 times a day. Urine is normal and bowels is also better. Should I stop predmet 4 , cardarone, freuselac ? czpam is stopped.Please advice.

Brief Answer: He has mild hypovolemia Detailed Answer: Hi again It is good that thyroid profile is normal. He can continue cardarone. Freuselac may be causing hypovolemic hyponatremia. It is a combination of loop duiretic and potassium sparing diuretic. Diuretics in general may cause loss of fluids (hypovolemia) along with electrolytes (hyponatremia etc). Since your father is now taking hypertonic ORS, he is getting better. His hyponetremia is mild, but if it worsens a change of this medicine will be required. Predmet is a steroidal medicine. I am not sure why it has been prescribed, so cannot comment on its usefulness. Hope it helps. Dr Vaishalee
