Question: hi, Iam 64years old and I have a complain of pancreatitis problem with difference in serum levels once in 2 or 3 months, either the amylase and lipase levels goes up, with severe abdominal pain, Iam sick with this problem since, I need to get hospitalised with huge hospital expenses,Can any one help me with a solution

Hi Sir,

I understand your concern.

You seem to have acute exacerbations of chronic pancreatitis.

This is causing you the fluctuations in serum enzyme levels.

At this point its extremely important that you assess the anatomy of the pancreas.

For this I advise you to get a CT scan of the abdomen.

This scan can accurately assess the ductal architecture of the diseased pancreas.

Further management is based on the test results.

Usually chronic pancreatitis with recurrent enzyme elevations and pain abdomen is managed surgically.Medicines will not be effective at this stage.

The need and type of surgery is assessed by CT scan.

Usually pancreatic drainage with or without resection is done.

Hi Sir,

This condition can be medically treated with analgesics and antibiotics only to a limited extent.

I generally treat my patients with oral tablets of Tramadol for pain relief and parenteral ceftriaxone as antibiotic. I suggest you to please see a doctor if his views are same and if can prescribe these drugs for you.

In case if you are unwilling for any surgical interventions,I advise you to get an ultrasound abdomen to document any innocuous gallstones,since many cases of pancreatitis are associated with gallstones .

Based on the size and location of stones either drugs or ERCP( endoscopic procedure) can offer permanent cure.

Hence I advise you to go for an ultrasound abdomen at least before stepping on therapeutic protocols.

Always quit smoking and alcohol if any.
