Brief Answer:
several possibilities can be there..thorough evaluation needed..

Detailed Answer:
Hello XXXXX, Welcome to HCM,

First of all i'm really sorry for the delay in response, it doesn't happen usually..

Yes, too many things you've gone through so far and that too at such young age, really don't have words to console, but i do believe that your an extremely strong lady, hold on, it'll get over, I'll help you as much as possible..

Yes, as you've rightly mentioned, there's several different medical conditions and some of them are prone to cause similar episodes like this..

You've had cervical bone replacement with an dead bone graft, do you mean ACDF ? can you please check for the details and let me know.. Also at which level it was done and from which approach (just let me know anterior or posterior)..

As per your descriptions, it's fitting in ACDF only, and after the procedure, it seems that you'd a barium swallow which indicated the esophageal narrowing..

Can you please tell me the exactly when did you have the Cervical spine surgery..and was the side ache started after or even before that ?

All these neck, back, side pain can be even inter-related, so please tell your Neurologist/Neurosurgeon in details so that a detailed physical examination is done and some relevant investigations to look for any residual nerve compression, disk herniation, scar tissue formation etc.

Can you please specifically locate the area- which part of the right side, is it right upper abdomen or even higher in the right rib cage area ?

It's not likely to be related to the ulcerative colitis though..however the colonoscopy must be done to look in to the internal colonic structures, so please go ahead with that..

Hernia is another possibility and more likely can be an epigastric hernia, but again, a clinical examination should be done to rule it out..

However, I'd strictly advice you keep the physical therapy restricted to only foot part, as until confirm diagnosis is obtained, physical therapy of this upper body part can actually aggravate and worsen it further (your PT instructor is right)..

The air hunger part is still unclear, I'd recommend a Chest X-ray to be done in the first place to look out for any mass/swelling grossly followed by a thorough check up by your treating doctor..

I'd like to concentrate on eliminating organic causes at first, rather than focusing on PTSD and other anxiety disorder, as that's a different entity all together..

Yes, a long list of complicated symptoms, but with proper directional examination, investigations , quite possibly treatable too..

Have faith in yourself and in your doctor, I do believe your in good hands and you'll get over this..

Let me know if there's any further query, I'll be glad to clarify..
