Posted on Mon, 12 May 2014
Question: For several weeks having palpating feeling in upper chest up to throat. Also jittery sometimes and left hand has mild tremors sometimes.often feels like something stuck in my throat. Do you think this is from previous thorasic surgery from esophagus problems or from scar tissue from thyroidectomy I had last year. I also have a little cough the last month

Brief Answer: Pleased, you need to be examined for more details! Detailed Answer: Hi and thank you so much for this query. I am so sorry to hear of all these symptoms that you have reported. They are not very typical and making a call as to what the exact cause is would require some more information. This would probably be obtained through a complete clinical evaluation and probably some laboratory investigations should more details be needed. Chest palpitations and tremors of the hand may signal a problem with the thyroid gland in the form of hyperthyroidism. What type of surgery did you undergo and what was the indication? was it for an over active thyroid gland? I will suggest that you get your TSH levels checked to know exactly what the state of your thyroid function is. Do you take any thyroid hormone supplements and if yes, what dosage? It will be unlikely to have palpitations and tremor as side effects of surgery or the scar tissue. These symptoms are more in relation to an excessive excitation which could be cause by hormones or in other cases anxiety, etc. Let's get the initial things done which are getting a full examination and a TSH level. Further investigations and treatment would greatly depend on these. I hope this helps. I wish you well.Feel free to ask for more information and clarifications should need be. I had thyroid cancer my TSH is .59 (.350-5.50) Total t3 6.4 (71.0-180.). Free t4 1.5 (.8-1.8). Thyroglobulin antibody 1.5 (.0-.9) can any of these numbers be why I am having these issues? I don't see my dr till later in May. So any suggestions would be appreciated

Brief Answer: When were these tests done? Detailed Answer: Hi and thanks for this follow up precision. When were these tests done? They look grossly normal but I would like to have more recent results if these have been more than 2months. Your symptoms make e concerned of a possible hyperthyroidism. Are you on any thyroid supplements? My thinking is that all the thyroid gland would have been removed and you put on thyroid hormones supplements. Please, clarify me on this. In all, get these numbers controlled if they are more than 2months. I lean towards excess thyroid hormones as a possible cause of these symptoms until otherwise proven. Thanks. Dr. Ditah, MD

Test done last week. Yes thyroid removed and yes because of the cancer the goal is to have a very low TSH. Don't understand term "grossly normal". T3 below normal range and throglubulin AB above normal. .... which is weird in that I have no thyroid. I'm taking synthroid 150 mcg Confused as to get"these numbers controlled" which you said were "grossly normal"

Brief Answer: See below! Detailed Answer: Taking a look at these numbers, non is so alarming. Just like you mentioned, it is weird you have thyroglobulin abs of which you are theoretically considered not to have any thyroid gland left. Maybe the remnants in your system before the surgery? But has been a long time and probably something isn't going on right. Synthroid is thyroxine and is T4. There is really no point in measuring these t3/t4 as they are totally regulated by us. The main indicator of how well the drugs are working is the TSH which seems to be normal though on the very low side of normal. T3 would of course turn out low because there are no supplements of t3 being administered! 150mcg seems a lot to me. If I were your doctor, I will not mind to drop this dosage and see whether there would be any change in your symptoms. There is really no point in getting these numbers controlled now as they are very recent. I will rather recommend that your doctor reduces the dose of synthroid and see what happens to your symptoms. Feel free to keep the discussion going. I wish you well and thanks for using our services. Dr. Ditah, MD.

Because of the high thyroglobulin anti bodies I had to have ultrasound of throat/thyroid bed. Tissue was found and now waiting to see what's next. Yes with the thyroid cancer they keep TSH as low as possible to reduce risk of reoccurring cancer

Brief Answer: Let's examine these facts! Detailed Answer: The TSH is kept at low vales because of the thyroid cancer? Thyroid cancer all by itself would keep the TSH very low because an active cancer would produce high levels of hormones(T3/T4) that suppress any significant secretion of TSH. TSH levels are normally used to monitor patients post thyroidectomy for cancers because very low levels may signal some active cells actively producing hormones that may explain this. For real with an ultrasound that showed some thyroid tissue, this means it is actively secreting hormones. Added to the 150mcg that you take daily, I am some what not surprised that you have presented with symptoms of excess thyroid hormones. If all the cancer cells were taken out, there would be no need on stressing to have very low TSH because there would be no reason to be afraid of. Also, cancer cells often do not depend on TSH to secrete high amounts but they are rather autonomous and that is the reason why cancers would result in very low TSH. Low TSH preventing cancer cells from multiplying and producing active hormones is not typical. The mere fact that you do not take parathyroid hormone/calcium supplements means all the thyroid tissue was not taken out because the gland that produces this hormone is embedded in the thyroid gland and the thyroid gland cannot be fully excised if these remain in place. My reasoning remains the same. I hope you get to find answers to these concerns and address them as soon as possible to regain your health. I wish you well. Dr. Ditah, MD.
