What Causes Dizziness With Heaviness In The Eye, Ear And Neck?

Question: hello doctor,I am XXXXXXX developer worked for 7 years on laptop but from past 5 months started having issues.I did stoop fwd while working but now tried adjusting my screen and body recently. I feel heaviness in the right (eye, ear, neck) and pain in the back side of the head on right side. I also feel itchy and heavy just below my right ear lobe. And when I lie down I feel very dizzy as things rotating when lying on my right side. I have suffering with this from past 3-4 months. I am not able to do MRI I am claustrophobic. I am in desperate need of guidance can you please please help me. I have visited ENT, Neuro, Ortho as well :( .Can a neck ultrasound help.

Thank you for your query.

1. From your description, rule out Benign Paroxysmal Positional Vertigo (BPPV).

2. A Video NystagmoGraphy (VNG) test will help.

3. Request a trial of medication with an anti-inflammatory medication and a muscle relaxant.

4. Many test for dizziness require direct observation. Upload the clinical examination notes of the ENT, Neuro and Ortho for my reference.

Thank you Doctor, I got my MRi done for cervical which shows straightening and stiffness in vertebral, Mri brain was fine, I visited my Neurologist he has put me on myospas, pregabid,paroxetine, for 15 days I dont knw when I dont lie flat on bed on right side things are still better , otherwise I get on with dizziness unable to balance myself.My right side of neeck is still stiffness from ear till shoulder.I still feel bit pressure in the right ear. When I lie on bed straight face up , and my right hand on my forehead , i feel something inside my right eardrum like some fluid moving drop wise , though I have got my endoscopy done twice its seems fine from outside .attaching the notes along.
1st attachment is of july first I visited neuro after cold I had these
2nd I visited ENT when I was on Vertin August
then side where better I had septalin and did some exercises of spondalytis and Vertigo
3rd I visited again 3 days back with the problem I described earlier....

Thank you for writing back.

1. Your symptoms and clinical notes are consistent with BPPV. This should be treated after a VNG test with re-positioning maneuvers and exercises. These include the Brandt Daroff Exercises, Semont's Maneuver and Epley's Maneuver.

2. Cervicogenic Vertigo is unlikely and should be treated with anti-inflammatory medication, muscle relaxants, local painkiller gels and hot fomentation.

3. An Impedance Audiogram will help check for any fluid in the middle ear.

4. With the above line of treatment, this should settle down soon.

Thank you so much for the guidance doctor, I will definitely get the Impedance Audiogram test done soon.

I have few doubts can you please let me know.
1) if I have vertebral arteries blockage ,can that be possible how will i come to know about that.
2)I tried Brandt Daroff Exercises at home I got so dizzy and nauseatic after doing that twice that I cannot even explain. I felt i was better before the exercise. Sir will I be able to overcome this, I am very scared now. Last time I remembered I itched my ear in sleep putting my finger inside for 5 minutes and next morning a got first time dizzy after getting up from bed.Can that be the cause. Will I be ever normal.
3) How should I sleep, if I elevate with big billow or on couch I get up better then lying flat on bed. As I also have stiff neck.
4) Can middle ear fluid cause so much vertigo and issues.
5)Why do have stiff neck since so many months.

Hi,

Thank you for writing back.

1. Vertibro-Basilar Insufficeincy can be detected with an MR Angiogram, and if this is not possible a Neck Color Doppler study can be done.

2. Dizziness is expected in the exercises. If the Brandt Daroff exercises are not tolerated, the other maneuvers should be done under medical supervision.

3. You can sleep with the head propped up on two pillows foor a few days, depending on your neck discomfort.

4. Middle ear pressure changes and fluid build-up can cause such symptoms.

5. Chronic neck stiffness can be due to many reasons including posture, normal ageing changes, cervical spondylosis and by the strain of keeping the head steady to avoid dizziness.
