What Causes Burning Sensation In The Abdomen?

Question: Hello Doctor

I have been experiencing some strange problem in my stomach. Almost since 14 days i am feeling a burning sensation in my stomach especially while drinking tea in the morning. Also when my stomach is empty i feel there is excess heat in my stomach. When i am drinking cold water i feel a cold sensation likely on the left side on my stomach. I consulted a doctor and he has put me on pantocid 40 mg on empty stomach for 1 month .. I have been taking pantocid since 14 days but the problem comes and goes.

My everyday routine
1)I sleep for 8 to 10 hours
2)I am also exercising at gym for 1 hour everyday
3)Diet consists mostly of fruits and clean foods devoid of fatty and spicy meals
4)Stress is there but cant interpret how much it too much and too less
5)I dont take any kind of drugs, alochol and smoke (No past record either)

I am really concerned about this problem because its hampering my life. I feel hopeless from inside. Please help me identify WHAT IS THE PROBLEM. Will i be able to be normal again. Please help me doctor

The burning sensation you feel in the abdomen, especially on empty stomach is due to acidity. Pantocid should have provided relief.

You have had H.pylori infection before and the treatment failed twice. Did you take rescue therapy for H.pylori as suggested earlier?
Was endoscopy repeated after that?
Was H.pylori eradication confirmed?

Hello Doctor

Yes i was on triple therapy antibiotics for 1 month .. After the treatment was over i felt okay and was able to eat without any restriction. When i had hpylori i used to get a burning sensation only while eating, i also had severe constipation but right now i am not having those kind of symptoms. I only feel cold when i drink cold water and when i am drinking tea i feel someone has lighted a matchstick in my stomach( but the sensation is not so strong). Also when my stomach is empty i feel there is excess heat in my stomach and flatulence is also there . What could these symptoms indicate and what should be the treatment like?

Thanks for the additional information.

As I said earlier, increased acidity is causing your symptoms. Since there is no relief with Pantocid, I would advise you to get an endoscopy done again.

As far as flatulence is concerned, some amount of flatus is normal. It is said a normal person may pass flatus about 15-20 times per day.
Increased flatulence occurs when the fiber content in your diet is high or if you have bowel infection.

If Gasofilm or Beano or Galto is available in your place, you can take it for relief from increased flatulence.

I am going for endoscopy tomorrow. But out of curiosity i would like to know what will doctor for during endoscopy. What makes me a liable patient for endoscopy. Do my symptoms also relate to GERD? . Please clarify

Hello Doctor

I had endoscopy and have uploaded the report. My h pylori is positive. i don't think initially it was eradicated so you can see the medicines prescribed in the report. Also i want to ask can this second line treatment mentioned in the report successfully eradicate the infection. What is all those red scars seen in the images? My follow up doctor told me nothing to worry, everything will be okay once infection is cleared. But still i am worried because i believe h pylori is resistant to all antibiotics be it levofloxacin or amoxicillin ... Please try to pacify my fear doctor because i am in dilemma. I feel as if i have lost all hopes of clearing the infection

Thanks for getting back to me with more questions.

Yes Levofloxacin based rescue therapy has eradication rate of 75-95%
In one another conversation I had with you in February 2016, I advised you to go for this rescue therapy.

All those red lesions are signs of gastritis. I told you in my previous post that the doctor would be looking for gastritis or ulcer. You have gastritis and this gastritis is caused by H.pylori infection as diagnosed by the Rapid Urease test (RUT).

I would advise you to go for 14 days treatment for H.pylori eradication followed by T.Razo for 2 months.

Once you complete the treatment get tested for eradication of H.pylori.
You can go for Urea Breath Test or Stool antigen test 8 weeks after completing the treatment.

I have few questions

1)What if this rescue therapy(second line medicines) also fails to eradicate the bacteria. What alternative do we have in future? I am asking this because earlier my treatment failed to clear the infection

2)I have heard gastritis is an incurable disease. We can only minimize the infection but cannot cure it permanently?

Please clear my doubts

1. It is wise to answer your first question after you complete your treatment and get tested again for eradication of H.pylori which I have mentioned in my earlier post.

2.Gastritis is not just due to infection. It can occur due to other causes also. It is curable.

Hello Doctor

I can understand your views but you know this disease is giving me nightmares. I read peoples reviews in Facebook groups that they have carried this disease for years and years. They were on and off treatments which ultimately lead to failures. It may be the fault of the patient but we should not ignore the limitations of medical science which says that hpylori is becoming resistant to antibiotics. This is why doctors dont guarantee 100% success of eradication. Suppose the treatment fails then what other options do i have in place? I request you to pacify my fear because i dont want to live with this disease throughout my life.

Hello Doctor

Okay i will wait till the end of the treatment. My doctor told me its not mandatory to go for retesting once course of treatment is finished. He told this because tests like 1)h pylori antibodies panel 2)stool culture dont give reliable results. They often lead to false positives and negatives. Therefore In order to confirm eradication can you tell me what accurate and highly sensitive tests are available?

You have to do tests to know whether H.pylori has been eradicated.
This is because you have history of treatment failures.

I mentioned in one of my earlier posts about what tests to do and when.
You can do either Urea Breath Test or Stool antigen test 8 weeks after completing the treatment ( Treatment is 2 weeks of anti H.pylori followed by 8 weeks of Razo).

Okay doctor. Thanks for your advice. One more question .. I am having discomfort in my left throat. It seems something is pressing against the tissue in that area. But there is no pain. I went to an ent and he gave me the following medicines to take for 3days twice

1)defcort 6
2)amoxyclav 625
3)seradic p
4)rabidoc d
5)tantrum mouth wash

I havent started the h pylori treatment .. So can i take these medicines first to clear the discomfort. Also i would like to know whats causing this discomfort and for what has the medicines been prescribed?

You did not mention about this symptom in your first post.
This could be either globus pharyngeus or pharyngitis.

You need not take those medicines.
You can start the treatment for H.pylori which itself will take care of the throat symptom.
