Question: Hello sir.

I have done TMT vand 2DEcho yesterday. It's all normal. Like this I am doing every month for last 3 years. I have some heart fobhia. Because my left hand and left side of neck including shoulder burning, pinching and tingling. It is very irritated. I am afraid of heart when it was irritating.
Please advice is it heart related or not

Hello, I am Dr Mody and I would be addressing your concern
Dear friend, i went through your previous question and having asked the same to other respected panel cardiologists, might be you still haven't got a closure y to your anxiety. Henceforth I will be brief and to the point.
1)for a 34 year old guy, the chances of heart issues is itself low, there is no need to get TMT or echo so frequently as it's not going to change. Generally a TMT has a warranty for 18 months to 2 years for a young guy like you. Also a exho will go abnormal after you have a heart attack not before, so again doing it so often is NOT going to help.
So how do we proceed.
1) The symptoms you describe occurs characteristically in cervical compression. Get a MRI cervical spine to rule that out on priority
2) think about your stomach, do you have bad meal habits , constipation, burping occasionally, symptoms increase post food, coffee, chocolates or spicy things. More when you sleep. Than think about reflux gastritis.
3) though nit medically indicated, get a CT coronary angiogram done, if normal, you don't have any blockages 99.6 %, you don't need an conventional angiography as it is Invasive and not indicated.
4) rbbb is physiological and is present in perfectly normal individuals
Also remember one thing anxiety will not prevent heart problem, rather increases future Risk, so try relaxation therapy and do visit a psychologist. He may help you tone down, you have got into a OCD loop with your heart fixation.

Nothing needed cardiac wise at present, however nxt step would be CT angio

Detailed Answer:
As all have impressed upon, nothing is needed is TMT and echo is normal and you don't have risk factors.
However TMT is true 75% time, so 1 out of every 4 we may miss. So one to rule all cardiac underlying condition, a CT scan of the heart vessel, which is called CT coronary angiogram, so if that is negative we should forget heart forever as it is 99% right.
Conventional coronary Angiography Is the normal angiography which h is routinely done, however it is Invasive procedure and your symptoms are too atypical to have one done.
Also more than any of the above MRI cervical spine would be the first investigation based on the symptoms you describe.
All the best for life and trust me you cannot prevent anything happening, though with good lifestyle you may decrease the chances of anything wrong happening.

Please give my final advice. Is it really required C T angiogram.
Because as per your answer again I am worrying why doctor suggested CT angigram. I am thinking any problem or your advice is to come out fobhia.

You don't need it medically my friend
It is too bring you put of your phobia because it's a non Invasive 30 second scan and not harmful in anyway, at the same time it will help you be sure that everything is good and you may go back to your normal life again.
So the final advice
NO TEST NEEDED AT PRESENT FOR THE HEART.

IF AT ALL YOU WANT EVALUATE, GOD FOR MRI SPINE

IF YOU FEEL SOMETIME IN FUTURE THAT IT'S THE HEART AGAIN, THAT I STEMI FOR ECG OR TMT, GO FOR CT SCAN OF THE HEART.
