Question: hello Doc
we had gone through IVF process on 3 XXXXXXX ( 3 embryo transfer )
on 13 XXXXXXX my wife was having serious trouble. her belly was fully bloated. she was fainting. unconscious, vomiting.
I admitted her to hosiptal in. icu for 3 days . her bp was low, feeble pulse.
I was fully aware that these are symptoms of OHSS post transfer which is mostly likely pregnancy.
on 16 she was shifted to general ward and her vitals are back to normal.
however she is in too much of discomfort . her hcg came to 230 on 16th.
I am worried about her health. as this is caused by ohss which is triggerred because of pregnancy her condition might get worsen because of rising hcg. pls advice on how should I take this forward. hosiptal will discharge by tomorrow. what should I do. how I can take are of her health. I will do my best to do ohss management.

tomorrow we are getting discharged and I want to understand how to manage rising hcg and ohss. is it going to be worsen ?

Ask her to have rest and no jumping even a brisk side change while lying down is risky.
Actually her ovaries are very enlarged so any such activity is dangerous and avoid sexual activity too.
You can discuss the addition of cabergoline tablet with your gynaecologist and have a prescription for that too.

Sorry for the delay in response.
I know this is the most common presentation and will take a couple of weeks to resolve.
Till then if urine output is adequate around 500-600 millilitres of urine a day and vomitings are less than 2-3 in number then nothing to worry.
Also ask her to lie in a little propped up position at an angle of about 39-45 degree of head side elevation of bed.
Continue all the medicines as advised.

Repeat bHCG after 48 hours

The rise is little less, but still it is a little premature to comment. So wait for another 48 hours and get it repeated and the values will indicate the viability of the baby.

Since the hcg levels are rising it is likely that it is a healthy pregnancy.

Yes dear this bloating will persist for a while and can cause slight discomfort to persist always.
She just needs to take rest as much as possible with normal walk once or twice a day at slow speed which will help in digestion and general wellbeing.
Most important is to take care of urine output which should be maintained to ensure she is recovering properly.
Ask her to have tea or coffee two to three times a day which will increase urine output.

Hello Doc
She spent one day at home . It was good. Wanted to share the details with you.

 Happy to come home after discharge. :)
I was measuring her vitals regularly
BP average (118 , 85)
Pulse 85, 115 highest( at night when she was in acute pain )
Spo2 93 till 96% - no breathing problem
Intake output was good. . Output was slight higher than intake. Urine was faint yellow.
At night she had acute pain with subsided in 15 mins. I had given her antacid and she felt relieved.
Girth 98 - 100

Anything to worry??

Few questions I have.
1. She is not able to sleep during night because of discomfort.  She cant stay in one position for more than 15 mins. She feels comfortable lying on one side. How I make her comfortable so that she can sleep.
2. How long this will last ?? As hcg is increasing and will increasing during whole pregnancy. .. how ohss will subside in 2 - 3 weeks ???
3. I will start giving her gaterade  ( sports drink ) for electrolyte balance.
Beta hcg will again do on 21st.. last value was 380.2 on 18th.

Will resolve in 2-3 weeks at least

She will take at least 2-3 weeks for resolution.
This relief will be because of regression of all the follicles due to atresia and thus hcg levels wil though rise because of pregnancy but the minute amount of progesterone secreted by each follicle and toxic fluids already present in the abdomen will decrease to negligible amounts and hence bring end to OHSS altogether.
All she needs now is abundant rest and antioxidants in the form of fresh fruits and veggies and energy.
Her sleep schedule will improve once abdominal girth stabilises but you can ask for a prescription of mild sedative from her gynaecologist tomorrow.
I wanted you to get bHCG test after 48 hours of the one done on 18th which was today but it's alright as the values tomorrow will also give us a lot of information.
Her girth has been around 100 since the very start and since it hasn't increased nothing to worry.
She can lie down I any position she feels comfortable as there is no problem in lying down at all.

Sorry I am again bothering you.
We did the hcg test today and the value came as 1306. On 18th it was 380.2
She is still struggling with ohss. Her girth is around 97 today.
Bp 118, 87
Spo2 94 95%
Pulse 100
Weight is reduced from 67.5 to 66.6

I want to know whether she needs to continue her medication that was given during ivf.?? Ambani hospital said just to take folic acid tabs.
I am attaching you the prescription from ivf clinic. Clinic Doc insist of taking these Meds,  but we both are very reluctant to take these and need advice whether it's really needed after hcg test.

You are welcome sir, I completely understand your problem as I deal with IVF patients daily.

Firstly let me tell you that it is a healthy pregnancy so nothing to worry in that direction.

I have seen the attachment and studied the prescription and would like you to continue all of them.

Only Lonopin injections can be omitted at this stage and can taper omnacortil tablets but frankly omnacortil has a healing role in OHSS and also will help in growing pregnancy.

Cavergoline is given just for OHSS and will be stopped once she is fine.

Most important is puregest tablets which has progesterone and is definitely needed.

Progynova can be omitted once in a while in case she gets uncomfortable after eating so many medicines.

So don't miss enrifol puregest, cabergoline and omnacortil.
