Question: Dear Doc,
This is the second time i am writing in Healthcare Magic.com.
my son who is now 2.5 years old, weighing 16kgs, has high temperature since last night, we went to doc and he has given us sinarist plus and Meftal p . SINARIST PLUS -6ML. and if the temperature is above 102, Meftal p 7.5ml....I am bit worried about meftal and the dose what he has given. so wanted to check once with you. (no offence to anybody here) being a mother and hearing too many things about Meftal...made me write you. hope you understand and let me know regarding the dosage and shoud we give meftal. or what is the other alternative.
now i have given him meftal 7.5ml as the temp is 102 , temperature seems have come down. but i am not happy . so Please request you to tell me. Thanks a lot for your time and Patience.

Hi...Thank you for consulting in Health Care magic.

What you heard about Meftal is right. I have even published a letter to editor in XXXXXXX Pediatrics, a renowned pediatric medical journal in XXXXXXX against using Mefenamic acid (Meftal-P). You can have a look at this at this link -

Meftal-P is an NSAID and contains Mefenamic acid, which is not recommended in children. It may cause drug induced gastritis and also renal problems on the long run. Please stop Meftal-P and never use it hereafter in children.

I will take this opportunity to alley your fears regarding fever.

Fever of few days without any localizing signs could as well be a viral illness. Usually rather than fever, what is more important is the activity of the child, in between 2 fever episodes on the same day. If the kid is active and playing around when there is no fever, do not worry. Fever is never going to be deleterious, but unnecessary medicines used for fever might have it.

Once viral fever comes it will be there for 4-7 days. So do not worry about duration if the kid is active.

You can treat fever with Paracetamol at 15mg/kg/dose every 4-6th hourly if fever is > 100F. I suggest not using combination medicines for fever as usually most combination medicines combine Paracetamol with NSAIDs and is not advisable.

Thank you so much for your immediate answer and also highlighting on usage of Meftal. I will stop it immediatly.
i wanted to ask you one question here, what is the alternative medicine for fever which is not coming down inspite of giving crocin. infact crocin does not work with my child. so is Imol or any other medicine which is helpful when the temperature is high.(>101, 102 ).
also what is the correct dosage of crocin DS to be given to baby 2.5 years old weight is 16kgs.

Please be specific regarding the fever medicines. as this information will be helpful to me in long run. temperature less than 100- what medicine and what dose to be given. and temperature more than 101 what medicine and what is the dose to be given. thanks a lot for understanding doc. i m tensed that is why i m asking you all these questions.

Hi....thanks for the trust in me...
Will come to your queries one by one -

1. Viral fever or in fact once any fever starts, initial 48 hours it will be difficult to control. But one thing I want you to understand very clearly - fever is only an indication that body is fighting out the infection and it doesn't mean that unless fever is brought down, then only the goal of treatment is reached.

2. Paracetamol is the best medicine for fever and I suggest you not to go for any other alternative medicines, as all of them are NSAIDs (you can refer in internet) and have deleterious side effects on long run.

3. For a kid weighing 16 kilos the dose of Crocin-DS (250mg/5ml) is 5ml every 4 or 6th hourly, that too only if temperature is more than 100F.

4. There is no such concept in medical literature / science that fever less than 100 this medicine has to be used and more than 100 this medicine has to be used. The directions are very specific - Give Paracetamol only if temperature is more than 100 and not below it. Less than 100 is not to be considered as fever.

5. For example if fever is 101, but the kid is active and playing around the recent literature says, not to treat that, as the kid is not having any discomfort due to that particular temperature.

6. Other alternative route of bringing down temperature is giving plenty of oral fluids and the urination will help bring temperature down.

7. Am always there to help...please do not get tensed at all. I have all my time to counsel to the parents to prevent even a single overdose of paracetamol or unnecessary use of NSAID drugs.

Thank you so much Doctor, you have clarified my doubts also made me aware of some of the medicines which we should not use.

I will only use Crocin 4 to 6 hours when the temperature is >100F.

I am not sure if i can ask you this (as i have asked one follow up question) but if possible please let me know , what if the temperature is more than 102, how do we control the temperature. the reason why i am asking you is my elder son who is 9years now, when he was only 1.7 years , he had high temperature and had got febrile convulsion, from then onwards, i really get tensed when my sons get high fever. so Please let me know so that i can follow the same. Hi,

I suggest you use only Paracetamol for fever. Even if it is more than 102 the advice is same. I agree with your concern about febrile seizure; but that febrile seizure is not related to temperature- sometimes it will occur at 100F and sometime it may not occur at 105F.

Do not do cold sponging as it may temporarily lower the temperature; but the rigors which develop due to cold sponging will again result in rebound high temperature. You can continue paracetamol syrup round the clock as I suggested every 4-6 hours. Remember to use the right dose based on the child's weight.
