Suggest Treatment For Severe Persistent Fever In A Child
Question: Hello doctor,
my son age 6 , 25kg weight, he is having fever 102F its 3rd day,
day1: we gave calpal oral suspension 10ml in 6 hour gap,
day2 : fever was continuing, in some interval, so we approached near by clinic, and doctor suggested below medicine
1. Mefenamic acid suspension 5ml : 1-1-1
2. Phenylephrine & Chlorpheniramine : 5ml : 1-1-1
3. Amoxicillin & potassium clavulanate : 5ml : 1-0-1
4: ondansetron 10ml, 30 mins before, taking food
Day3(today): we continued as prescribed by doctor from near by clinic,but today he is having 7 times loose motion(diareah,greenish yellow & very watery), and 5 times vomitted, fever is also not dropped,
I doubting something is not correct and doctor mentioned mefenamic acid suspension is replacement for calpal to drop fever quickly, but looks it's not working, I doubt it's not looking fever controlling medicine,
Please suggest the best to recover

I have gone through your query and understand your concerns. Let me mention that the greenish diarrhea that you observe in your child is due to antibiotic associated diarrhea. Most often the fevers are due to viral infections where antibiotics are ineffective. All they do is to eliminate the normal intestinal bacterial flora and thereby cause such diarrhea. Since you have started the antibiotics, you must complete the course. Also give ORS (oral rehydration solution) to make up for the lost water and fluids. Give plenty of curd or yogurt (alternatively probiotics) to replenish the intestinal bacterial flora.
Be patient. It will still last for a few days.
It is not due to mefenamic acid or calpol that the diarrhea is occurring. Continue to give them as needed. Also give tepid sponging.

my main doubt is fever is not controlling with mefenamic acid suspension, so what should we do, shall we go for calpal 250mg suspension instead of mefenamic ? if yes how much ml should be given per dosage?

The fever may continue for a few days. It is if the fever lasts for more than 7 days that you need to get concerned. Till then give both paracetamol and mefenamic acid if necessary. The doses can be given independently of each other. The appropriate dose for Calpol DS (250) for your child should be 7.5ml per dose. If Calpol is given, the dose should be 15ml per dose.
