Suggest Treatment For Severe Abdominal Pain While Weaning Off Omeprazole In A Child With Chronic Constipation

Question: Hello Doctor,
My 5 year old son is suffering from acidity and constipation for the past 2 and half years, i think the constipation is there from his birth. I have raised this query and Dr. Sumanth Amperayani was answering my questions, now i am not able to reply to him directly. My son is in Omeprozole and laxative now, we are trying to reduce the Omeprozale dosage and he is getting the pain on and off. when he takes 10mg that time he was completely fine (no pain) even though he was constipated. now we are slowly reducing the dosage as he is taking Omeprozole for the past 2 years. whenever we tried to reduce the dosage he gets severe pain and we are back to the original dosage. so this time we are reducing the dosage very slowly like reducing 1mg in each week. we have taken all the tests except endoscopy and colonoscopy in which doctors are saying its not required, the results are normal. now how should we get rid off these medications? we are trying home remedy now as per Dr. Sumanth Amperayani suggestion, it works most of the time but I cannot say a single pain free day, he is getting the pain every day 10-15 mins and about 4-5 times. he is getting relief when we give luke warm water/gentle massage in his tummy/ginger with honey + lime water/ a walk for 5-10 mins. Hi thanks... I was very happy to see you review in which you mentioned that you were seeing improvements.

I want you to realize a fact that, a chronic problem like constipation which has been there for 5 years will definitely take months to clear completely.

In your case pain is not because of acidity and it is very often due to constipation. I appreciate your gesture of coming down on the Omeprazole step by step, but according to medical literature it need not be done unless it is a steroid or a seizure medication. Omeprazole is not both.

The home remedies I suggested will work but slowly. The main issue which has to be concentrated in your kid's case is relief of constipation which is the base of all this pain.

I suggest you follow all the behavioral modification therapies I suggested in my previous interactions, so that ultimately it will be over and your son will become normal.

Please remember that it will take time for the complete recovery. It may take weeks and even months. Please do not get disheartened.

Hello Doctor,
Thanks for the response, I have the below questions on his diet
As we are not able to give green vegetables (he don't like to eat) we are making soup including carrot, beans, spinach on daily basis. Taking these in the form soup is effective?

We started giving raspberries, XXXXXXX apple (at least one of them) everyday with oatmeal, wheat bread and XXXXXXX food like idly, rice with dal,spinach and other cooked vegetables
Are the above food habits be sufficient for his daily fiber in take?

Hi...these are sufficient but I want you to cut out on soups and also carrot and potato do not contain fibre. Best is leafy vegetables and likes of beans, and other gourds. XXXXXXX is excellent. But Papaya I seven more good daily basis at night.

Thanks for the response, just curious why do we need to cut out soup? do you mean cut out the carrot from the soup? we tried giving Papaya in the night (just 2 pieces) as per your advice 2 weeks before, but he got stomach pain on that night so we thought of Papaya is too hard digest thats why he is getting the pain, so we stopped giving Papaya.

can you please also answer the below Qs
whats the amount of fiber he needs to have in his diet daily?
cooked vegetables Vs raw which one is good, we are giving cooked one only
drinking juice Vs raw fruits

Hi...answering your questions one by one

1. Soups do not contain much fiber
2. Stomach pain may not be related to papaya or its working and due to bowel movements he is having pain which will go off in due time.
3. He needs on average 25-30 grams of fiber per day but you cannot calculate like this and is difficult to administer also. Best is to take good amount of fruits and vegetables.
4. Raw is good but if it s unpalatable for him, cooked ones will do.
5. Juice doesn't contain fiber, best is raw fruits.
