Suggest Treatment For Pressure In The Sinus Area

Question: My 17 year old daughter has a lot of pressure in the sinuses above her eyebrows. She has two cysts in that spot that ebb and flow the pain, but are not the source of the problem. She had a CT scan - it was clear. No allergies. Tried Flonase - did nothing. Now trying a non-allergic sinusitis medication - not sure if it is doing anything? Pressure is very bad - sometimes blurry vision, can't keep her head up. Needs to sleep it off - doesn't always works. Wore her retainer two nights in a row, and that caused the pain to be worse. Any guesses what this might be? She has had the problem of years - thought it was just because she goes to sleep late.

Symptoms you describe are more likely related to sinusitis, or sinuses drainage dysfunction.

Worsening of pain after wearing retainers suggests that there is a mechanical problem such deviation of nasal septum and in this case sinusitis medications don't work well.

If the septum was found deviated on CT scan, I think a surgical correction may be necessary.

An ENT consult is necessary to understand better your daughter's condition.

If sinuses related conditions are excluded by an ENT Doctor, those symptoms may suggest changes in intracranial pressure and, in this case, a brain MRI is necessary to evaluate this possibility.

Thank you. We have been to an ENT physician, the devoted septum is minimal. She has had this problem her entire life, most likely. She felt great on Saturday after taking two dosages of the medication for non-allergic sinusitis, but after wearing the retainer, she went down hill. A lot of pain and pressure. She started anti-acne medication at the same time as the sinus med. She has had 3 bad days in a row - much worse than usual.

So, I don't think it is a change (emphasis on the change) of intracranial pressure because she has had this problem for years and years. Is that correct thinking?

Yes, it is correct thinking since medication for non-allergic sinusitis worked.

This is an element to consider as one of the diagnostic tests for sinusitis.

Changes of intracranial pressure for example should be considered as one of possible conditions responsible for those symptoms, if sinusitis is excluded totally.

Chiari malformation for example may result in such periodically change in symptoms.

Don't get me wrong, I'm not saying that your daughter is suffering from Chiari malformation, I think that it is necessary for a correct diagnosis of your daughter's condition and exclusion of other possible conditions is one of the steps to diagnosis.
