Suggest Treatment For Persistent Pain In Urethra

Question: I have pain in my urethra after I pee. I have trouble walking sometimes. After numerous tests including labs, CAT scans, MRIs.....they cannot find a cause or prescribe a treatment. Currently, I am seeing a pelvic floor specialist here in XXXXXXX For the past 6 weeks I have had infusions of pain medication into my bladder with little relief. There is absolutely no sign of infection or of any "structural" issue.

In August, I suffered from Acute Renal Failure likely due to a virus.

Please.....what direction, what questions or ideas might I pursue next.

Also, it is not a GYN issue...been there as well.

My Primary believes that this is a nerve issue.Someone suggested to my Mother that this could be related to MS? I plan to see a second opinion from another Pelvic Floor (GYN) Urologist. But, that appointment is not until Mid-November. The bladder infusions have helped some but, the improvement is minimal.

Hello Gdoolittle and welcome to HCM.
As an Urologist,i must assure you that, you could be back to normal in few months.
Don't get so worried and upset, as that alone will delay recovery.
Believe in your Specialists doctors like, Urologists, Nephrologists and Gynecologists.
The symptoms you mention, are typical of urethral inflammation.
Forget MS and nerve problem.
You should get urine routine and culture done, and blood sugar.
Also mention your last kidney function tests reports(urea, s.creatinine,uric acid).
Just as you seem to have recovered from acute renal failure, give this time.
Kindly avoid Ibuprofen, and bladder infusions for pain relief henceforth.
Tramodol occasionally is fine. Start drinking an alkaliser syrup twice daily.
Many find great relief, drinking fresh lime juice twice daily.
As your abdominal CT and MRI scans are normal, don't be worried at all.
You can stop the pelvic floor exercises, and do cycling, swimming, or jogging.
Hope your STD profile reports are also normal, VDRL, HbsAg, HIV.

I'm a bit confused. I've had 2 cystoscopys and they showed nothing at all!!!! There is nothing apparent about my urethra. I've also had ultrasounds and everything is negative.

You must explain this to me.

There's a condition called 'IDIOPATHIC", which means , no cause made out.
That's the reason i reassured you, there's nothing to worry about.
The only criteria for this condition is, all tests must show no identifiable cause.
Hope your doubts are cleared and are more relaxed now.
If you feel like, you may discuss this with your Urologist, or any other doctor.
This applies to all systems of the body.
This is the reason, i advised you to stop the infusions and start exercising.
Music is a great way to relax, as is yoga and meditation.

Thank you again for your response.
But, I want to check one more thing.

Yesterday, I was in pain when I woke up. As authorized by my doctor, I took 2 ibuprofen. In the early afternoon, I went for a walk. My bladder felt ok. When I went to pee, it felt like shards of glass. The pain is an 8-10 every day especially when I get up and move around. My Primary just prescribed an antidepressant.

Do you still have the same advice?

Ibuprofen isn't safe in your case.

You'll be confused, if you seek advice from several doctors.
Follow one who's advice you think, is from experience and expertise.
Ibuprofen is not safe in those who've had renal failure, and that's the reason i asked you to avoid it.
Hope you've stopped the infusions and started exercising.
I'm advising you from experience, after treating patient's as an Urologist.
You should check your s.creatinine, as you've been taking Ibuprofen.
Antidepressants are of temporary relief only.

You should be able to re-confirm, if Ibuprofen is safe, in view of your previous renal failure.
I still advise you to avoid it, as regular use of Ibuprofen itself can cause renal failure.
