Suggest Treatment For Persistent Nausea, Cold Intolerance And Bitterness In Mouth

Question: I am struggling with my health for last six months and have met several doctors. However, I don't think anything is working for me. Here are few symptoms that I have.

Sensitive to cold temperature. As soon as I enter Air-condition (mainly in office), I started feeling very cold after an hour even though I wear warm cloths and jacket.
Feeling of some thing radiating from my left feet. Some time warm and some time cold. When I am in cold environment (AC), my full body feels chills. In normal temperature, I don't feel cold but my left feet feels radiation sensation. It feels like some thing (cold or warm) is radiating from my left feet and feet fingers.
I feel nausea quite often. Although I have never vomited but I feel like vomiting. This is better if I take PPI (Nexium 20 mg)
I feel a bitter/soar taste in my throat (root of the tongue).
Feel like sleeping in after noon.
Feel tired.

For nausea and soar/bitter taste, doctors have suggested that I have GERD. However, for sensitive to temperature and sensation in feet, none have diagnosed anything yet. Please note that my thyroid (TSH) is normal. I have got it tested twice.

Hi!

I'm Dr. Neel Kudchadkar, from Health Care Magic, I have read your query carefully and being your doctor, I will take care of your health issues.

You have mentioned that you have a sensitivity to cold, and you experience unwanted sensations in your left foot.

I have noted well, all the tests you have performed, including Thyroid Functions.

I will now request you to take action on three things:

1. Kindly repeat the Thyroid tests, And this time I would ask you to note
"anti TPO antibody titres" This is sufficient. I am also aware that you have taken
this test twice, however, thyroid problems can occur at a lower level.
And cause these problems in you. We will take it from this point onwards. I am not sure if your doctor has ruled out subclinical hypothyroidism. Please ask them if they have not. This is where the TSH can be in normal limits between 3 to 5.5 but some symptoms may appear.

2. You might need to test your blood, all round checks to be performed,
particularly for "iron deficiency" and "acanthocytosis". Please note these 2
terms carefully while getting the blood test done.

3. I would wish that you describe further, these issues, of cold sensitivity and
a good description of the "radiating" sensations, that you feel in your foot.
Actually, this will help me, in the diagnosis, and plan an effective treatment
for you.

4. If you feel all of the above were already ruled and there is no meaning in repeating then the next thing I could suspect is functional symptoms where there is no label in the form of diagnosis.

Thanks for looking into my issue.
Both the time I had my thyroid test, TSH levels were low (first it was 1.8 and second time it was 1.4.). T3 and T4 were also within normal range. Do you still suggest that I might have subclinical hypothyroidism? Should I still go for Thyroid test noting "anti TPO antibody titres" ?

I have tested my blood couple of time and all the counts were within normal range including hemoglobin and cell counts. Do you still prefer me to go for these test again? If yes, what blood test should I ask the diagnostic center to perform?

When I am in normal temperature environment, or in hot weather, I don't feel cold. However, during day time, I feel like something is radiating out from my feet and feet fingers. It feels like feet has become warm and heat is coming out. So the sensation itself can not be described. I feel nervous and tired. My eyes burns and feel like sleeping.

When I am in cold weather or in Air-condition, even though I put lot of warm cloth and with shoes, I feel very cold within inside. Feet and leg becomes very cold. I feel very nervous and feel very tired. My eyes burns and feel like sleeping.

Hi!

Ask for a Hemoglobin estimation in the blood test.
If there is less hemoglobin, take either an Iron injection or iron supplement tablets.
Ask for this "Acanthocytosis" in the blood test also.
If this is found, see a hematologist and take it further from that point.
Though this is a rare finding, usually.
You might want to redo the Thyroid tests.
Kindly see an endocrinologist.
