Suggest Treatment For Hyperthyroidism During Pregnancy

Question: hello,my wife is 28 weeks pregnant and his thyroid reports are like this tsh:-0.06 t4total:-28 and t3total:-1.91....is there any problem in reports...he is taking thyronorm 125 mg once a day as suggested by her doctor...

she is taking 125 mg since last 2.5 months and sometimes she feels paplitations....his previous reports when she was 24 weeks pregnant was like this...tsh:-0.11 and t4:-14.28....so the dr suggested the same 125 mg tablets...

Hello sir, can you please send me the scanned reports of both 24 week and 28 week.
And I would like to know if she was taking thyroxine before 24 weeks?
I'm concerned because in my opinion the prescription is wrong in the first place when she was given thyroxine on TSH report of 0.11 because normal values of TSH in pregnancy are 0.4- 2.5 IU.
I would like to have a quick feedback from you so that I can guide you at the earliest because the reports stated by you are suggestive of the fact that she is going into hyperthyroidism which is not good in pregnancy.

actually her treatment was started in january her reports was like this tsh:-64 and t4:-5.91...and dr suggested to take thyronorm 50 mg once a day...and then after one month she got pregnant and dr said to take 100 mg once a day....and after that in the next report tsh was 1 80... some time she take 100 mg...and now from around 2.5 month she is taking 125 mg...

Hello sir after reading the sequence of events I would recommend her to take 100 ug of thyroxine instead of 125 ug and also get TPO antibody levels tested to see for autoimmune disorder of thyroid gland.
Also after getting the report visit an endocrinologist and discuss the case so that he can examine her and see for signs of hyperthyroid status and keep a regular follow up as recommended by him.
Thyroid levels are maintained by this titration method only so regular follow up has paramount importance.

Hi, till now everything is fine but proper titration of dose is very important for smooth delivery of baby.
There is not going to be any harm as of now but in case she stays uncontrolled in terms of thyroid hormone levels then she will suffer during labor and immediately after delivery.
So visit an endocrinologist as soon as possible.
Till then she should take 100 ug of thyroxine.

After reading all the reports it's clear that she is fine and her deranged thyroid hormone levels were all due to the escalation of dose from 100 to 125 ug.
Since you have switched back to 100 ug her thyroid hormone levels will be fine soon and all you need now is to repeat the hormone levels after 4 weeks from now and we can discuss it then.

No sir hopefully everything will be normal baby will be safe because bad effects of thyroid hormone are more in early months of pregnancy.
Moreover hyperthyroidism is not very bad for babu in utero as it only causes harmful effects during the time of labor.
And drug induced hyperthyroidism has a better prognosis than spontaneous hyperthyroidism.
Just ask her to keep taking her medicine daily and keep a regular follow up with your doctor.
