Suggest Treatment For Fever, Tonsilitis And Dry Cough In A Child

Question: Hi Doctor

I have a nephew who is 3 years old, he has fever for almost 5 days now, on his 2nd day with fever i checked on his pharynx and tonsils and he has really enlarged pharyngitis/tonsilitis . so i decided to get him Zithromax 200mg and i gave him the dosage based on his weight as it shows on the syringe which was 3ml for his weight (12kg).

it's been three days since i started giving him zythrmox and he is better with fever now but he still has enlarged tonsils/pharynx that i can see it above his throat by just looking at it. and he developed dry cough yesterday

today is his 4th day and i'm still not sure if i can continue giving him zythromax because as i learned in medical school it says we can only give zythromax amoxicillin for only 3 days, can i continue giving him for 2 more days ?
what are the risks of continuing with zithromax?
what do you suggest??

Welcome to the HCM

I understand your concerns but don't worry. Most likely cause of these symptoms is a streptococcal throat infection. Zithromax (Azithromycin) if started should be given for 5 complete days at 10-12.5 mg/kg/day once daily to prevent further rheumatic fever episodes in future. This is not commonly associated with any significant side effects.

Also, the tonsils are slightly more visible than in adults during 2-8 years of age. So, do not judge your treatment based on size. Congestion and systematic symptoms like fever should be monitored.

Since your nephew has responded to Azithromycin as per your description, I would recommend you to continue it for 2 more days.
