Question: Hi am curious as to why my ALP test is climbing all within normal range. Always was around 97. Since XXXXXXX of last year went to 106 then to 103 then to 118 then to 115 now 127. Could this be from Vit D3 3,000 I.U. And baby aspirin. Both of which I just started taking a year ago. Or possible this last jump to 127 I had a bad contact dermatitis and was on a steroid cream for three weeks

Brief Answer:
Elevated ALP as a result of bone metabolism

Detailed Answer:
Thanks for contacting HCM

You are concerned about having an elevated alkaline phosphatase over the past year. Your elevations are only marginally elevated and not at dangerous levels. Alkaline phosphatase is an enzyme that is found in the bone, kidney and in the liver. This enzyme elevates when one of these areas is having increased changes or damage. To be sure where the elevation is originating from I recommend that the ALP be fractionated. This is where they can break down the ALP and determine its origin.

When you mention you have been taking large amounts of vitamin D for osteoporosis this may artificially elevate your ALP. With vitamin D it allows the bone to rebuild and release ALP during the rebuilding process.

It is unlikely that the aspirin is causing the elevation. I also do not believe that the recent use of topical steroids will elevate ALP. Large doses of oral steroids can cause changes in ALP but since steroids are only marginally absorbed through the skin you were not exposed enough for the ALP to elevate.

Therefore I recommend that you have your ALP fractionated. If not then repeat your ALP again in three months. I also recommend reducing your vitamin D to 1000units daily along with calcium to help you rebuild bone. I also recommend stopping any alcohol or Tylenol as these can cause liver damage and again elevate your ALP

Hope I answered you question. Please contact us again with your medical questions and concerns.
