Hello, rate of dormant HPV is very high to the tune of 60-65% but to stay dormant till 20 years is less likely as many triggering factors can cause active infection to occur.
So only less than 5% affected patients can have it dormant for a period of 20 years.

Could you have a high-risk hpv show up on a pap smear exam and also have the genital warts one all at the same time after 20 years? These are two different strains, correct? This seems too cooincidental.

Yes both are usually different strains but at times pap smear gets contaminated from the skin flora which can give fallacious results.
You need regular pap smear examination and use condoms everytime you indulge in sexual activity.

I have had pap smears each year for the past 15 years and they have come back normal. The one I just got back showed up abnormal so they did an HPV test that came back "high-risk." The OBGYN did a procedure where she took a biopsy. Since it has only been a year or so that I have had a normal pap smear, would it still be possible that the biopsy would come back as cancer?
