Suggest Treatment For Clostridium Difficile Colitis And Strep Throat

Question: I am currently in the hospital positive for reoccurence of c. diff colitis, severe and sudden onset (w/I 24 hrs) strep throat, (strep is positive) 15 day early beginning of menstrual cycle. However I am concerned that the daily change of hospital it's are missing something. Ct scan 2 days ago showed enlarged spleen, blood panels came back with high RBC, low WBC, low MCV, low MCH, and low lymphocyte %. 40 year female with 38 year history of Crohn's disease, Graves' disease diagnosed in 2012, now post surgical hypothyroidism, lupus diagnosis 2013. Blood panel abnormalities have been consistent for over 1 year along with debilitating fatigue and pain. Crohns is in remission, however severe flank pains near kidneys and pancreas in stride with blood panel abnormalities and other above complaints. Now seeing much blood in urine. Everything gets written off to crohn's related. But I know deep down something else is wrong. Take no maintenance meds except 275 mcgs Synthroid. Tsh too high and free T4 too low still after thyroid removal in 2012.

Well i have gone through your history and i am a little confused here.

1. At present you are admitted for Clostridium Difficile Colitis right?? I would like to know were you taking any antibiotics prior to admission which has triggered this admission?

2. I suppose your Crohns and Thyroid are under control but you have mentioned low blood counts along with splenic enlargement. I would like to know here, are your Total Leucocyte count, Hb and Platelet low( all 3 lines ) or is it just the WBC. Splenic enlargement with the low counts needs to be looked into.

3. You said you have been diagnosed with Lupus but i dont see you are on medications for it. Lupus can explain most of the abnormalities you are having. SO if you could throw a bit more light on it, it would be helpful, like the symptoms you were experiencing along with any Antibodies which was positive.
It is quite essential here as Lupus needs to be controlled.

4. Regarding the blood in urine, i would like to know what exactly your urine examination shows and the Renal function tests. It can very well be due to Post Streptococcal Glomerulonephritis or due to Lupus as well.

I feel there are a few loose ends here which needs to be tied for us to common to a final diagnosis.
