Question: This question is for my wife, who is aged 38 years. She has been complaining for intermittent pain on the shoulder tendon, and sometimes suddenly it radiates down. There is a specific place exactly behind the shoulder on the joint of shoulder blade. But the pain is not persistent, it might appear sometime. Doing work does not increase it. Lying down turned left increases the pain. Sometimes just lying down straight and waking up in the morning shows the pain. She has not taken up any medicines for it so far, just going by home remedies. Initially one doctor suggested taking calcium, a year ago. But after that there is no improvement. On looking through the internet, the symptoms appear to be of tendonitis of shoulder but not sure. Please let me know how to proceed with treatment. She does not take pain killer as stomach upsets.

Brief Answer: Tendinitis,cervical rib,cervical spondylitis Detailed Answer: Hi Dear Welcome to the XXXXXXX It all appears to be tendinitis. Means inflammation of muscle end where it is attached to the bone. Many times this inflammation develops due to over and unusual use of that particular muscle . This can be over come by some regular physiotherapy, antiinflammatory drugs along rest to that muscle. Usually it takes some to subside. Orthopedic consultation to be sought for local examination, exact diagnosis and proper management. Physiotherapist consultation is quite important for the symptomatic relief Xray cervical region with shoulder is required to reach the diagnosis. Other possible causes may be extra cervical rib. Cervical spondylitis etc which may be causing some irritation of cervical nerves and in result pain and its radiation. Hope the query is answered. I have been being treated for achilles tendonitis for the past month but continue to get worse. The orthopaedic PA put me in a walking boot, told me to elevate and ice the tendon, and to stay off of it as much as possible. I have worn the boot and followed the other directions to no avail. Now the swelling will not go away even with ice application and elevation, there is severe pain and bruising to the back of my knee and I have muscle spasms in my calf at night. I just had an MRI done last week, but now my doctor is on vacation for a week so I will not know my results. What can I do to stop this pain and swelling and to heal this?

I have had chronic tendonitis in my shoulders since I was 15 (I m 26 now). I am used to taking anti-inflammatory medication (ibuprofen) o-t-c for the pain that I am in all the time. In the last few days, the pain in my right shoulder has gotten unbearable. Ibuprofen hasn t worked, heat rubs haven t worked, sleeping upright/flat on back/properly on side hasn t helped. Resting it or moving my arm--everything hurts. I went and got X-Rays at the ER because the pain was severe. I overhead the x-ray techs talking about calcium, but the doctor told me my x-rays were normal. She then ended up prescribing Tramadol 50mg to me despite the fact I take Adderall XR 25mg. It s a good thing I asked the pharmacist about potential drug interactions, but shouldn t the doctor have had the sense not to prescribe a pain medication that could interact with my Adderall and cause seizures? I ve had to have steroid shots in my shoulders before for acute tendonitis pain like this. Even the pain medication and anti-inflammatory (Ketorolac 10mg) are barely dulling the pain. Instead of the 8-9 I was sitting at before the visit/medication, I am fluctuating between 5 and 7. Is there anything I can do at home to help relieve the pain I m feeling that I m not already doing? I ve been alternatively resting/gently stretching my arms and alternating hot/cold on the shoulder + using the medications prescribed. Also, is it possible that I have developed calcific tendonitis and the doctor I saw in the ER was incompetent? The fact that she prescribed interacting medications w/o a second thought makes me think that she really didn t try to help me at all. The pain was too severe to wait to schedule a normal doctor visit or I would have done that. Should I have insisted on a corticosteriod shot to reduce the inflammation? Even now, with both medications (the anti-inflammatory and pain reliever), my tendons feel like they are on fire.
