Suffering From Frequent Urination. Diagnosed As Prostatitis. Took Medicines. Ultrasound Normal. Help?

Question: Hi
I am 21 years old and 5'10"
From the last 1.5 years i have been urinating very frequently.the problem is off and on..for some days it goes away and then comes back again.the stream is also slower than normal.when i masterbate within seconds it feels that something has filled up and i have to ejaculate.
I have been to the doctor as well.he gave me medicines citing it as XXXXXXX prostatitis.while i was consuming medicines it got normal for some days but came back few days later after i stopped taking medicines.i got ultrasound done and it was normal.
Please tell me what to do.it is very frustrating and depressing.

Thank you for your query.

The first thing to think about is prostatitis, prostate enlargement or urinary infection but since this lasts for so long and treatment wasn't useful, it should be further investigated. Also prostatitis isn't common at your age and any urinary infection including prostatitis is considered to be complicated when found in young males.

It means that there might be some underlying cause which facilitate infections. This can be congenital anomaly, obstruction of ureter,bladder or urethra or some other urogenital disorders.

Ultrasound can visualize only prostate and for other urinary functions you should do intravenous urography or endoscopic procedures. Till that do kidney function tests and urine analysis to verify is there bacterial infection which can be treated by certain antibiotics then. Also, drink enough fluids,limit masturbation, avoid XXXXXXX purine and XXXXXXX cholesterol food.

This is not likely to be something XXXXXXX

There may not even be any particular cause. If this is only infection and prostate enlargement then it should be treated by antibiotics and prostate drugs till symptoms subside. There can be some urine flow disorder too and then you should be treated by diuretics. So most of it can be treated by medicines, but consult your doctor about this.

Premature ejaculation isn't easily treated by medications unfortunately. There are only 2 options which are considered to be helpful: antidepressants and topical anesthetics. Although, its efficiency isn't great.

There are also Kegels exercises, sex therapy, psychotherapy, and some methods like pauses during the intercourse or slowing it,using double condoms, purposefully ejaculating before sexual intercourse etc.

I am sure some of these would be helpful.

I wanted to tell you that when the doctor told me that the problem
Is prostratitis and gave me the medicines.I had the medicines for around 10-15 days and during this time when i masterbated there was no semen.but the erection got better and i could hold on for longer as well.but at the climax there was no semen.my urine problem got better.but when i stopped the medicines there was semen when i masterbated but urine problem again came back
Them after 1-2 months i went again to the doctor to get my urinating frequently problem fixed and he said it may be an overactive bladder.i had those medicines for 5-6 days but the problem of urinating frequently got worse.so i stopped it
Now what should i do?

Hi again,

You should do further tests. There is no point of guessing what it is or giving you various medicines and see which one helps. Your doctor shouldn't say what it may be, if this is overactive bladder it should be proved, prostatitis should be proved too. If nothing is found then there can be functional neurogenic disorder or psychological problem (which isn't uncommon cause).

Since you have persistent difficulties then the right diagnosis should be established no mater how unpleasant those tests might be. So if you want to be sure about the diagnosis you need to consult specialist urologist and do micturating cystourethrogram (MCUG). Frequent urination in this age and ejaculation problems aren't something rare but it shouldn't be ignored.

Also, you should take medicines till end when started. Antibiotics are mostly used for 10-14 days. I don't know what other exactly medicines did you take, but it is not advisable to stop medicines before time that was prescribed by the doctor. I still think this might be unhealed cystitis or prostatitis and I would rule out these things first.
