Suffer From Psoriatic Arthritis. Had Cortisone Injections And Oral Medicines. Have Severe Neck Pain. Suggest.

Question: I am suffering from Psoriatic Arthritis since 8 years. I have had cortisone injections and Oral medicines as well.

Still my pain has not reduced. Due to my Arthritis, I am unable to attend to my daily routing works as well.

I now have severe neck pain (Referred as Spondylosis) and pain (No Movement) in the left wrist (Referred as Synovitis).

I need your help in bringing down my pain as all medicines (Repeated) by many doctors (Rheumatologists) have worked only for short period.

I am a diabetic as well. The medication that I take are:

SAZO 1000 (2 times Daily)
FOL 5(1 Time Daily)
METHOTREXATE INJ 0.8 (Saturday)
METADOZ IPR 500 (2times)
AMARYL 1 (2times)

Even after taking these medicines, my arthritis is deepening day by day. MOrning Stiffness is too heavy. I am not able to lift my 2yr old daughter. Hope you understand my concern and will help me out.

I am Dr. Vaibhav Gandhi, I am Orthopaedic surgeon .

I have studied your case.

You are taking good amount of allopathic medicine to control your arthritic state
newer injection are effective in controlling arthritis include infliximab,denosuzumab, anakinara .But since you diabetic as well you need to be taken under guidance of specialist doctor. This is not an easy task for the treating doctor as well. There is a lot of participation involved by you as well. Do not leave everything for the medicine to take care. It should be a holistic care by Rheumatologist, Physio, Psychologist, Dietitian.

Exercises-
Do spine extension exercises,lying on your stomach lift leg,then lift only head and chest,
Do stretches and rotation of all joints

Here are my other useful suggestions to you:
avoid cold water ,weather, AC, cooler
Avoid lifting weights
Sit with a support on your back.
Take break every half hour while at office. Try to do some stretches and joint movements


If the above do not help, you may need to see a Physiotherapist/yoga teacher
Check your vit B12 level and vit D3 level through blood test.

Stress also increases arthritis and stiffness so I advice:
Get ample sleep, good amount of water and fluids .eat fresh fruits. Tackle stress at right time, don’t drive bike for long time,keep your body weight in normal range.

Thank You Doctor, I need some more advice.

I have undergone wax treatment for wrist and electrco magnetic therapy for neck. I used to feel better after the treatment. But after some days again the problem pops up.

My Doctor had adviced to have RETOZ 90 Mg (SOS) in case of extreme discomfort.
I feel very comfortable after having this medicine. I have started to play badminton in the mornings. Excercise of the neck is also done by me. My diabetes is under control. Now as the rains have started in Kerala, i am forced to stay indoors in the morning. I do some stretching excercises, but due to extreme discomfort, i am unable to continue. After having bath in hot water, i feel more relaxed. I go by car to office. I am a chartered Accountant working with XXXXXXX Airtel Limited in Kerala.

Can you suggest me a very good specialist for my case in Cochin, please... In case you live somewhere in south, I am ready to travel and XXXXXXX you and take your treatment.

My personal relations with my wife are also being spoiled because of my discomfort to joints.

Hope you can understand and will help me.

If you are not able to go out for badminton, you should do in door daily exercises like sun salutation, yoga, etc.

You will have to explain to your spouse about your disease so that she will help in improving morbidity, you may need counselling session with expert.

I have seen good results with a combination of yoga, breathing technique, meditation, etc. You can learn it in Art of living Part 1 course.

Thanks for your response. I had met my rheumatologist today. The dosage of sazo has been increased to 3gm daily. Defcort 3mg in the night and all others remain. Biological were also discussed. I come to understand that the medicine is costly. I need to know other possible options available. Will Ayurveda help. Doc has asked me to report in 2 weeks. Chest xray has been taken to show the radiologist before the biological injection. What r the side effects of biological. I'm afraid I amnt ready for this probably bcoz of the cost and also bcoz of the side effects. Can you refer me some medication to bring down my stiffness. I know that I am having good amt of allopathic medicines. I also do exercise daily. Just that I need to be a bit comfortable from stiffness. Pls help me

Continue your medicine.

For releasing stiffness take physiotherapy.

Ayurvedic oils can release stiffness for some period but it can reoccur after some time.
