Question: Hello dear Doctor...
I have some troubles with my health and looking for advice as for me it is a very scary to go to hospital... I afraid doctors, have a syndrome of white coat and always looking for symptoms of different diseases due to viral hepatitis in my childhood and bad memories...Cant do anything with that...
So, some general info here,
Male, 33 year old, 180cm, 94 kg, no weigth loss for last 10 years. Appetite is same as always, can eat everything. Never had alcohol before, started to drink 3 years back but again, once in 3-4 weeks.
I am oilfield expert, office based now, but used to travel a lot for the last 7 years. Have a stressed job for the last several years, Moved to France recently, and after couple of month I noticed that after each lunch (regularly had meat and fried stuff) I start to get my stomach expanded, had a burp and gases. Was feeling myself slightly weak, slight itching im my eyes, higher sensitivity to light. Those symptoms disappeared after one month, all gone, I back to normal, was ok for the next two and have got another discomfort, in colon, left side just below the belly. Got panic attacks very often. Gas, some mucus in stool (no blood noticed), some particles of undigested vegetables, stool itself is not fully formed. Might be medium soft piece followed by soft. No constipation at all, stool is regular, once or two times a day (when discomfort slightly elevated. I cannot say that I had or have a pain, it's just dicomfort).
Was adviced by my colleague to check the liver and did that, bilirubine - 17, ALT-78, AST - 79, GGT - 91, TSH, Amilase are normal (do not remember exact figures, but was in range, according to table was attached), B, C hepatits markers are negative.
Could you please help...?
Thank you in advance.

Dear XXXX,
Thank you for contacting XXXXXXX with your question.
You are a 33 year young man and as per your own
description , under a lot of stress at work.
Your weight is O.K. for your height and it is good that
you are maintaining it.
I looked at your blood results and it appears that you do
have a mild case of Hepatitis. You mention that the tests
for Hepatitis B and C , came back negative which is good.
That leaves few other conditions that we have to rule out.
First of all , even though the frequency of intake of alcohol
is not daily , the amount that you may have been consuming
could affect your liver. Initially , it causes Fatty liver and then
progresses to advanced liver disease and cirrhosis.
You need to get a sonogram of the liver and gallbladder which
can also rule out gallstones that could give you some of the
abdominal symptoms that you are experiencing.
You can have irritable bowel syndrome which can give you the
changes in the stool consistency , bloating , gas and abdominal
pain.
You need to have a complete lipid profile and HbA1C test to rule
out diabetes. High lipids are also associated with fatty liver.
Include in th your blood tests "Celiac Panel ". This can give you
various stomach symptoms.
Make sure that you have a high fiber diet with whole grain cereal,
fresh fruits and vegetables. Take vitamin supplements , especially
Vitamin- C 1000 mgs. daily, Super B- Complex one daily ,
Vitamin-E 800 i.u. daily . All these help to heal your liver.
Most of all , you should stop drinking alcohol . Stick with Red grape juice.
I wish you the very best.

Doctor,

Thank you for your fast response.
Just couple of questions more. Sometimes I feeling myself hot, checked my temperature but it was 36,7 evening time. Apart from this recently (once blood analysis came out) I feel some pain in my finger joints or legs (no visual changes)
Is there any relation with symptoms I previously described?

Hello, XXXX,
You seem to be describing mild arthritis symptoms.
Especially , if the smaller joints are involved , have to
think about Rheumatoid Arthritis. It can occur in your
age group. I suggest that you also include in your blood
tests ,immune markers such as XXXXXXX Anti-DNA, R.A.Factor,
Thyroid and Anti smooth Muscle antibodies, Serum Complement
level. This would help diagnose or rule out the conditions.
I wish you well.
