Is TSH Level Of 5.10 A Cause For Concern?

Question: Hi,

I am 25 weeks pregnant currently, I had TSH checked yesterday and it is 5.10. I am worried as it is high. Currently i am talking half a tablet of thyronorm 12.5mcg from monday to friday and full tablet on saturday and sunday.
Please let me know if abnormal thyroid levels effect my unborn baby before and after birth. If yes how does it affect and what can be done for that.

Welcome to HCM. I have gone through your questions and understand your concerns.

Your TSH at this gestational age should be below 3 miu/l. So you have to increase the dose to 25 mcg daily. You can recheck the total T4 and TSH after 1 month.

Don't worry. Hypothyroidism of this level won't affect your baby. Hypothyroidism will increase the risk of abortions, preterm delivery, bleeding and infections in mother. So if your baby is born term, he/she will be normal.

You just need to take medications regularly to control hypothyroidism.

Thanks for your reply doctor. I have read in some blogs that the baby will have less IQ compared to others if the mother is suffering from hypothyrodism. And the babies need thyroid from mother till 12 weeks. By the time thyroid test are done most of the time is gone. So how does this effect the baby.

And as suggested by you earlier shall i increase the dosage to 25mcg per day.Currently i am talking the tablet in the morning during fast and give half an hour gap. Should i continue the same?

Hi Mrs XXXXXXX

Trust me, nothing will happen to your baby.

Baby usually start producing thyroid hormones only after 20 weeks. Till then thyroid hormone is required for proper implantation and development of fetus. That is the reason why abortion rate is very high in untreated hypothyroidism. If your baby has progressed to 25 weeks now, that means he has received enough thyroid hormones from you.

Even in untreated severe hypothyroidism( like TSH >100) , the IQ difference from babies born to mothers without hypothyroidism is less than 7 ( not a big difference). This is not your case as your TSH is only slightly elevated. So stop worrying, your baby's IQ won't be affected by your hypothyroidism.

More importance now given for thyroid diagnosis and treatment, is not because of IQ change in baby, but since it is an easily treatable cause of abortions and preterm delivery.Your TSH is a risk for abortions and preterm delivery, not for an IQ change in baby.

You can take the medicine as you weer taking now.
