Is Akurit 4 Safe For Liver?

Question: Hello Dr,
I have been diagonised with TB PCR during a laproscopy surgery for ovarian drilling. I hv been put on akurit 4 since may 21st'14. I read many posts here that it causes liver problems and Iam now worried. Does it really harm liver ? How often should I get LFTs done to keep a check on it ?

Thanks for writing to HCM

Akurit 4 contains four antitubercular medicines and it contains Isoniazid ,Rifampicin,Pyrazinamide and Ethambutol.

Before starting Akurit 4,patient should have baseline serum uric acid and liver function test like SGPT and SGOT(SGPT is most important).

Liver damage is due to Isoniazid,Rifampicin and Pyrazinamide.Monthly assessment of SGPT should be done throughout the treatment.

In patients with liver dysfunction,monitoring of SGOT and SGPT should be done every 2 weeks throughout the treatment.

Then what is the alternate medicine to akurit 4, which does not cause liver damage and also should be equally effective in curing TB PCR as akurit4. ?
In case I need to continue akurit4 what are the precautions to be taken in form of diet and additional medicines to safe gaurd liver

Thanks for writing back

Drugs contained in Akurit 4 are the first line drugs for Tuberculosis.
Other medicines are advised in resistant T.B.

You shouldn't be so worried.These are highly efficacious drugs and it should be tried first.First you need baseline SGPT and uric acid so that any adverse effects can be compared later on.In most of the cases of liver toxicity,only dose adjustment is required with continuation of same drugs.

You should high protein diet.
