How To Test For HIV?

Question: Dear Doctor
Ten Days back i had sex with with a lady in legal brothel in Singapore and then next day I received a handjob from a Lady from street walker .I used condom during sex and during handjob i didnt use condom .Now i having mild fever , a small swelling on my left forearm and pain on lower neck back side .I am worried that i infected with HIV .Can I do any test to confirm the status .I got to know that we need wait for three months to get the correct result .Is there any test which i could take now so that i will get peace of mind .Any test centers in XXXXXXX ..

I will try to help you

The best option in XXXXXXX will be to go to KMC hospital. I know as a fact that they manage HIV there.

As for the tests, besides HIV, there are many other STDs that can be transmitted. And we will need to evaluate for some more common STDs as well.

I would suggest that you meet a local specialist and start what is known as post exposure prophylaxis. This is a short term therapy of anti HIV medicines that are prescribed so as to try to avoid transmission.

As you know and have correctly mentioned the conformed tests take minimum 3 months, you can still do a couple of tests initially. They may be falsely negative but there is no harm in doing them. Any ICTC centre in government hospital in XXXXXXX will do these tests, usually free of cost- they have their own protocol so I wont be able to tell you what tests they do.

Since you have used the condoms, and I am assuming there was no break in the condom, I am quite hopeful that there has been no transmission.

Thanks Doctor for your reply ...I am worried because i am having mild fever ,soar throat,couple of red marks on face ,swelling on my forearm ,jaw pain below ears and
minor tongue infection .I got to know that these are early symptoms of HIV ..
Could you please let me know whether i will have the early symptoms within 10 days .I was using condoms and it didn't break .Can i go for testing, whether it will getting the correct information now .

I am glad to hear the confirmation of what i was expecting.

I think there is no harm in going for the tests. Its absolutley fine.
Of course, as you said, the tests may be a bit premature and thereby may be falsely negative. So you will have to repeat the tests at a later date. But at least you will know initially that you are ok, specially since you have some symptoms.

Of course, I should explain to you also that these symptoms can be due to another cause. A simple URTI - throat infection can cause the symptoms that you have mentioned. And as I suggested, since you have used a condom both the times, I feel you will be ok. Chance of HIV transmission is very, very low in such cases.

Well HIV centres in govt sector are more reliable and are more secretive about the identity. They dont even inform the reports to doctors suggesting the tests. Besides post exposure prophylaxis (PEP) is usually more easily available. Still if you want to go to a pvt lab, you can test HIV DNA PCR test.

Hi Doctor ,
I got to know that there are two HIV PCR test, which one shd i take ..HIV DNA PCR or HIV RNA test.Do i need any letter from doctor for the testing .
I am feeling better , Fever and cold are gone ,now having a little neck pain ...How long will the symptoms of early HIV persists .

Hi, I am glad to hear that you are better. I have always been telling you the this doesn't look like hiv. Early symptoms of hiv can last a few days. The symptoms are dependent on the type of infection that you develop.

It's important to realize that in hiv one basically catches common infections. It's just tougher to heal them.

I think I would suggest that you test hiv dna pcr if you still want to go ahead with the tests. Letters are usually not needed in the ictc centre of govt. The private labs may ask for a letter depending on their policy.

if you are not worried, normal schedule is good enough

Ideally one is supposed to get the tests done at 3 months and 6 months and 1 year. If the test is negative, all the three time, 99% chances are that there is no HIV. Since you are feeling better, I suppose you can wait for the scheduled period and then get tested as per the normal schedule.
