How Long Should Epilex Chrono Be Taken For Seizures?

Question: My sone had siezure (jerky movements of limbs,tongue bite and but no frothing from the mouth) in night sleep at 11:45 pm 7 months before (Oct'15). We took him to hospital in emergency and they did MRI, CT scan, EEG, and all tests were normal. However Vid D: 3.1 and B12 was 271, Low Calcium... Doctor prescribed Levilex for 2 months and stopped but Epilex Chrono 300 (2 times per day) is still continue. Other medicines were given - Cap HexaD3, Cap Corus, Cap Ortocal...Last week D3/ B12 test done and now D3: 92.1, B12: 1452. Doctor has stopped Hexa D3, Corus, Ortocal but asked to continue taking Epilex Chrono 300 twice a day....during last 7 months, my son has never had any such siezure or any type of issue....How long epilex chrono 300 shud take? Was this really epilepsy?

1. Following an Episode of Fits, Anti Convulsants are given for 6 to 18 months depending on the nature of fit. The investigations shall be repeated then and if they are normal, the drugs are slowly tapered (dose reduced gradually) and stopped

2. Single Episode of Jerky movements of limbs and Tongue bite alone may not constitute Epilepsy. We have to observe for few more months and see if there is any more episodes. Also, as mentioned above, we need to Repeat the Investigations and if they are normal, slowly withdraw and stop the drugs

Thanks for your response…
Doctor who is doing treatment of my son has mentioned in his prescription "clinical diagnosis - Fit IGE (JNE) Hypo vit D3", this FYI...

Kindly answer following queries:
1) What is cause of above diagnosis?
2) Vit D3 was very low (3.1 only) when checked after major Fit (such seizure was for last about 2-3 minutes) had noticed? Can low Vit D3 be reason for such fit? My son had also noticed some incidents when he was carrying books but suddenly books fell down and also he noticed some minor jerks in rest room (only in morning time). He told to Doctor during investigation when he got admitted in hospital after major fit..He never told to us (parents- Me or his mother).
3) He is 16 yr old and planning to go to USA for higher study after 14 months. Is it ok if he can travel and stay alone there? His mother got scared due to last major Fit in Oct’15. And never came any such minor or major such seizures any more since Oct’15 till date and his medicine epilex chrono 300 (twice/day) is continue.
4)You suggested to repeat investigation before stopping medicine after 18 months so what type of investigation should be done – CT, MRI, EEG or blood test?
5)Is any side effect of Epilex Chrono 300 if he consumes upto 18 months? In Oct’15, LFT/KFT/CBC tests were done and were ok. Before starting his medicine.
Should we do any blood test now as he has been taking medicine since last 7 months?

I will answer one by one

Q 1) What is cause of above diagnosis?
A 1) Idiopathic Means No Reason can be attributed. Generalised means no specific part or body was affected. Epilepsy mean Seizure disorder. Hypovitaminosis means low level of Vitamin D3

Q 2) Vit D3 was very low (3.1 only) when checked after major Fit (such seizure was for last about 2-3 minutes) had noticed? Can low Vit D3 be reason for such fit? A 2) Low D3 can cause fit. But whether there was some other factor which added to that seizure needs to be evaluated

Q 3) He is 16 yr old and planning to go to USA for higher study after 14 months. Is it ok if he can travel and stay alone there? His mother got scared due to last major Fit in Oct’15. And never came any such minor or major such seizures any more since Oct’15 till date and his medicine epilex chrono 300 (twice/day) is continue.
A 3) He can travel. But he should follow the following precautions till next April
a. Normal day to day physical activities.
b. Food: Normal Diet, but Carbohydrates should be restricted.
c. Drugs : as prescribed by your neurologist
d. Follow the instructions of Dotors / Your Family Physician for other ailments
e. Daily bathing, Head bath allowed. Bathroom / Toilet Door Should not be locked from inside
f. Not to step into Wells, Ponds, Lakes, Falls
g. As far as possible, avoid Locking the room from inside when he is alone in the room. Try to get 2 way lock
h. Not to drive
i. Not to use use sharp instruments

Q 4) You suggested to repeat investigation before stopping medicine after 18 months so what type of investigation should be done – CT, MRI, EEG or blood test?
A 4) If there are no further seizure episodes, CT Brain Plain and Contrast, MRI Brain Plain and Contrast, EEG including Sleep Study, Blood Tests have to be repeated in April 2017. If there are any further seizure episodes, these tests have to be done again immediately after the seizures

Q 5) Is any side effect of Epilex Chrono 300 if he consumes upto 18 months? In Oct’15, LFT/KFT/CBC tests were done and were ok. Before starting his medicine.
A 5) Epilex Chrono does have side effects. But Side effects of fits are much more. Hence, in spite of side effects, we need to continue the drugs, till we are sure that there won't be further fits

Q 6 ) Should we do any blood test now as he has been taking medicine since last 7 months?
A 6 ) He has to do Complete Blood Count, Renal Function Tests, Lipid Profile, Liver Function Test, Vitamin D3, Vitamin B12, Thyroid Profile Now. He has to do again in coming October 2016 and next April 2017

I have reviewed the results

1. Glucose, Lipid Profile, RFT, LFT done on 12/05/2016 are within normal limits
2. Thyroid Profile done on 12/05/2016 is within normal limits
3. Vitamin D3 done on 30/04/2016 is normal and Vitamin B12 on the same date is high. B Complex Vitamins will get excreted if they are more and hence there is no need to worry
