1. Decreased urine output due to renal failure secondary to alcohol induced cirrhosis occurs in 40% of cases. Its a condition called as hepatorenal syndrome.
2. I am not sure, at present, it is hepatorenal syndrome, as liver failure is not advanced enough. But could be early stages. Hepatorenal syndrome, once diagnosed is life threatening and progressive.
3. At present inadequate hydration, low protein and fiber in diet could be responsible for decreased urine output and constipation.
4. Ammonia levels of 75 is elevated. Difficult to correlate it with confusion as ammonia levels alone are not the factors in diagnosis of hepatic encephalopathy.
5. Vodka needs to be completely stopped, as alcohol induced damage, if ongoing will lead to further liver damage. Liver transplantation would be the only choice, if further damage occurs.
6. Need to get counseling, psychotherapy and avoid alcohol or any hepatotoxic ingestion. Hospital stay, would help in getting professional help.
