Hi Doctor, I Have Been Having Really Weird Erection Problems

Question: Hi Doctor,

I have been having really weird erection problems for 4 months now. It started back in March of this year when I realized I had phimosis my entire life. Up until this point, I thought that an uncut penis looked like mine and wasn't supposed to retract. When I found out about this I felt really conscious about my penis and has very sad that I never new about retracting my foreskin for 26 years. I went to the doctor and he prescribed me triamcinolone cream which I applied to my penis for about 5 days while doing streching excersizes. my foreskin was able to retract over the glans on the first day that I applied the cream. About a few days later I noticed that I woke up and didn't have a morning erection which was odd since I always have a morning erection for as long as I could remember. Then a few days after that I noticed that my penis felt "dead" and "numb" while I was driving to work. This instantely made me panic and I was stressed out and anxious all day at work. When I came back home I instantly tried to masturbate to see if I was still able to get an erection and I wasn't able to. On top of that, I ejaculated in literally 10 seconds (with a flaccid penis). I slept it off. I went to urologist the next day and he dismissed it as being due to anxiety. He didn't take any tests, he just did a physical examination on my penis and said that it is not possible to be impotent at 26 years old. He prescribed viagra and I did 1 pill that day and went off to have sex with a friend. I was able to get an erection but I felt like the viagra did not help and that I would have gotten the erection regardless. I ejaculated prematurely during this instance as well. Mind you, up until this point in my life I never had ejaculation or erection issues. Ever since then, I have went and seen other doctors and requested blood tests and ultrasounds but all they did was take a blood test. Everything came back normal (normal testosterone, blood sugar levels, etc.). They saw that I don't have diabetes, peyronies disease, or any other history of serious medical conditions, or the fact that I am not on any medication and he dismissed my issues being due to anxiety as well. I did bring up my morning erections to him and told him that some days I wake up with one and other days I don't and that i am concerned that this may be a physical erectile problem rather then a pyshcoloigcal one and he told me that it is nothing to worry about and that I should just meditate and try to get rid of my anxiety. I've had sex multiple times after this situation and each time I was able to get an erection naturally although I do not think my erections are as "hard" or rigid now. Also, sometimes when I masturbate to pronography now I am not able to get an erection and I sometimes still do ejaculate prematurely even during masturbation. Is this all due to anxiety? This has been on my mind literally every second of every day for the past 4 months and I am really feeling hopeless that all of doctors are not willing to do an ultrasound to see if maybe one of my penile nerves or muscles are broken. What if I have a vein in my penis that is leaking? Sometimes when I masturbate I lose my erection quickly now (it never happened before). Also, when I do get monring wood I lose it within seconds of waking up whereas before I would have the erection until I got up and went to the bathroom to urinate. If my erection issues were really due to anxiety wouldn't it only be related to performance anxiety? Is the fact that I am thinking about my erections so much causing my brain to not give me regular erections as before? I've had anxiety before with work, school, family ,etc. but never had any erection problems because of it. Do you think this started because of my realization of my phimosis and is no getting worse because I (wrongfully?) disganosed myself with some type of erectile issue? If this really were a psychological issue wouldn't I only have erection issues when I had sex and not during masturbation, morning erections and premature ejaculation? What are the chances of a 26 year old having physical issues with erections vs psychological? I know that the older I get the more my erections will decline but this seems like a HUGE and sudden decline whereas everybody else would go through this process gradually and not noticeably. The only thing I was able to find on the internet that may be effecting my erections would be constant pornagrphy viewing. Is this proven? I do watch pron often and I read online that watching too much pron will cause ED in young men. I am just so sad right now and I can't consentrate at work, I am very easily irritable now and just very annoyed that I can't get erections like I used to. Can you please advise?

i agree with your doctors for a healthy young adult male the common cause of erectile dysfunction is psychogenic in nature this is correct in 99% of the time exceptions would be patients with known disease of the penis that has caused damage in the vessels what causes erection.

you are suffering from anxiety towards the male organ and it's function and it's causing counterproductive effect on your erections. i suggest seeing a psychologist that specializes in sexual functions because i feel that you may need them more than a urologist.

for my advice i suggest you take deep breaths relax and don't mind the male organ in the meantime and once you're able to do this directions will go back to normal. i agree with limiting exposure to phornographic materials because multiple studies have shown they cause sexual dysfunction in both men and women.

And I now you said that 99% of the time in young healthy adults the erection problem is due to psychological effects but I am worried that MAYBE I might fall into the 1% of young healthy men who may in fact have a physical cause of ED. Even with phimosis, I read that more than 99% of men can retract their foreskin all the way back by the age of 18. That means at 26, I am already wayyyy less than 1% of men with a foreskin issue so I am afraid I may also be less than 1% of (healthy & young) men with a physical ED issue. Is it possible that the stretch exercises I am doing with my foreskin caused some type of nerve or vein damage to the penis? A week before this erection problem first started I even forced my foreskin back as much as I could on my erect penis and the the tight foreskin was very tight around my glans when I ejaculated. Would that have caused any damage to my penile nerves or veins as well? Plus I have seen about 4 or 5 different doctors and all they did was take a basic blood test. How would all 5 of them know just by looking at me that my erection problems are due to anxiety when they haven’t even done an ultrasound of cat scan on my penis to see if am something is wrong? If it really were a penile vein or nerve issue would that cause other symptoms as well such as pain?

My foreskin would never retract over the glans before i used the steroid cream and after I used it my foreskin retracts all the way down while flaccid but still does not erect al the way down while erect. When I have an erection my foreskin only retracts past the glans and not down the shaft. Would stretching it more make it more retractable? Is it possible that my foreskin can only be strecthed enough to retract over the glans not past the shaft?

also...what would be symptoms of I injured my corpus spongesium? I used to be able to be able to feel it and "pump it" from what I remember but now it seems to be weird.

I'm sorry. I know that there is practically no known reason for me to have physical problems with my erections since i am young and healthy and don't have any vascular disease that I know of but I also do find it a bit unrealistic that the change in the quality/feeling of my erections is due to anxiety since I have been through episodes in my life before where I had anxiety due to my boss, my job, family etc. and I never had erection issues arise because of it. Also, the lack of morning erections is also worrying to me because it's not like I could experience anxiety in my sleep therefore I would wake up with an erection every single day if my penis was mechanically working correctly...not just 5 days of week or something. Plus it's not like my anxiety is that bad to the point where it would prevent blood supply to the penis. Yes, I worry about my erection problem A LOT. Numerous numerous times throughout the day but when I am worrying about these problems its not like I am having rapid heartbeat or sweating or any physical symptoms of being anxious. I am just really worried that in a few years i won't be able to have any erections at all since I am already having these problems right now. And the anxiety that I found online related to ED is performance anxiety...which I don't have. I am not nevous or anxious about how I am performing in bed. I now see what my doctors were talking about when they refused to do further testing on me because I still do get erections just not as frequently and with more arousal needed. I am 27 years old and up until 5 months ago I had absolutely no idea that uncircumcised men were able to retract their foreskin. I never was able or even tried to retract my foreskin all of these years because I never knew that we were supposed to. I applied the cream on the tip of my penis and did my stretching excersizes with q-tips and, to my XXXXXXX I was able to retract my foreskin past my glans on the very first day (something I wasn't able to do before). I stopped doing the streching excersizes after about a week. I started doing it again for about another week but stopped again because I think I was stretching too hard and it caused some painful tears on the ring of my foreskin. I am able to retract fully with a flaccid penis but can only retract over my glans when my penis is erect. When I have an erection my foreskin becomes so tight right after the glans of the penis that I am unable to retract any more.



If I do more stretching and apply the cream will my phimosis completely disappear? Or is it possible that my phimosis can only be corrected half way? Since I was able to stretch it this much to have it be completely retracted while flaccid is it possible that it won't stretch anymore while erect?



also, I was also wondering why causes phimosis? It made me very insecure especially reading online that phimosis is extremely rare and that 99.9% of little boys are able to naturally retract their penis. Why would I be in such a rare category with my foreskin?

Eczema or psoriasis in the foreskin and in the penis is related to scar formation thereby might be causing the fibrotic ring in your foreskin. You may have a condition called lichen sclerosis in which there are white plaques in the penis and the anus and this may cause phimosis and other penile deformities. We suggest seeing a urologist so you may be able to do a complete physical examination and may advise you on what to do.

There is improvement with the conservative treatment of stretching and the steroids so there is a good chance that you will be able to dilate hit to the point wherein it can freely be exposed on the erect state.

I see...
I don't get enough sleep as I should but I definitely wouldn't say I sleep less than the average adult. I also am not stressed out or have anxiety while sleeping (or at least i don't think I do). So by that reasoning I do think it's kinda weird I am only 27 and not getting morning erections.

But I guess I just have to accept this as a psychological cause even though I do think it is strange that my morning erections decreased around the same time that I also felt the numbness in my penis/testicles/anus/groin area.

Another symptom I've been noticing is not being able to maintain an erection as long as I used to. It seems that the second that I no longer feel any arousal my erections go down very quickly and that's both while having sex and masturbating. Even when I do have morning erections they go away very quickly as soon as I wake up. Before, I used to be able to hold it or it would stay longer before going down.

I really would love to know 100% that this is all in my mind but all of these changes in erections are a little too coincidental. I understand that as we get oder the quality if erections change but I don't think that they should change in my 20's....and if they do I definitely don't think they should be drastic enough for me to notice. Correct me if I am wrong but it would be more of a slow and gradual/unnoticeable change.

I don't get enough sleep as I should but I definitely wouldn't say I sleep less than the average adult. I also am not stressed out or have anxiety while sleeping (or at least i don't think I do). So by that reasoning I do think it's kinda weird I am only 27 and not getting morning erections.

But I guess I just have to accept this as a psychological cause even though I do think it is strange that my morning erections decreased around the same time that I also felt the numbness in my penis/testicles/anus/groin area.

Another symptom I've been noticing is not being able to maintain an erection as long as I used to. It seems that the second that I no longer feel any arousal my erections go down very quickly and that's both while having sex and masturbating. Even when I do have morning erections they go away very quickly as soon as I wake up. Before, I used to be able to hold it or it would stay longer before going down.

I really would love to know 100% that this is all in my mind but all of these changes in erections are a little too coincidental. I understand that as we get oder the quality if erections change but I don't think that they should change in my 20's....and if they do I definitely don't think they should be drastic enough for me to notice. Correct me if I am wrong but it would be more of a slow and gradual/unnoticeable change.

Yes, you are correct in saying that it should be gradual and not as drastic as what you have experienced.

They still believe you have a psychogenic problem with terms of your erections and based on the type of your questions, you seem to be the type to be worried a lot with numerous things and since erection is the focus of your attention that's why you have experiencing problems with it.

Like I said before it should be better for you to see psychologist that specializes in sexual medicine.

At this point, I don't really think a test is necessary since physically I do get erections. Maybe sometime they aren't as rigid as I want and not as frequent as I want but there are few times where I still do get very hard erections like before. So I guess physically my body is still able to have erections. I will be going on a 2 week trip later this month so hopefully I can distract my mind and not think about my penis for a while and we'll see if everything comes out good.

In terms of the phimosis though. Let's say that the stretching doesn't work. Would I absolutely still NEED to be circumsized? When my penis is flaccid I can retract all the way down to wash my penis (and I do this daily) so if I can't retract all the way day down when erect would that cause any fatal diseases or likeliness of having penile disease/cancer in the future?

To my XXXXXXX when I retracted for the first time earlier this year my penis was not dirty at all. But now it is super clean.

as long as you are properly cleaning your organ when it is flaccid then the risk of having infection or cancer is very low and is the same as for a circumcised man.

if you can live with the foreskin not be able to fully retracting erections then you don't need to do a circumcision. primarily the circumcision is for you to improve your quality of life not dealing with pain during erections.

Hi Doctor, I Have Been Having Really Weird Erection Problems

Hi Doctor, I have been having really weird erection problems for 4 months now. It started back in March of this year when I realized I had phimosis my entire life. Up until this point, I thought that an uncut penis looked like mine and wasn't supposed to retract. When I found out about this I felt really conscious about my penis and has very sad that I never new about retracting my foreskin for 26 years. I went to the doctor and he prescribed me triamcinolone cream which I applied to my penis for about 5 days while doing streching excersizes. my foreskin was able to retract over the glans on the first day that I applied the cream. About a few days later I noticed that I woke up and didn't have a morning erection which was odd since I always have a morning erection for as long as I could remember. Then a few days after that I noticed that my penis felt "dead" and "numb" while I was driving to work. This instantely made me panic and I was stressed out and anxious all day at work. When I came back home I instantly tried to masturbate to see if I was still able to get an erection and I wasn't able to. On top of that, I ejaculated in literally 10 seconds (with a flaccid penis). I slept it off. I went to urologist the next day and he dismissed it as being due to anxiety. He didn't take any tests, he just did a physical examination on my penis and said that it is not possible to be impotent at 26 years old. He prescribed viagra and I did 1 pill that day and went off to have sex with a friend. I was able to get an erection but I felt like the viagra did not help and that I would have gotten the erection regardless. I ejaculated prematurely during this instance as well. Mind you, up until this point in my life I never had ejaculation or erection issues. Ever since then, I have went and seen other doctors and requested blood tests and ultrasounds but all they did was take a blood test. Everything came back normal (normal testosterone, blood sugar levels, etc.). They saw that I don't have diabetes, peyronies disease, or any other history of serious medical conditions, or the fact that I am not on any medication and he dismissed my issues being due to anxiety as well. I did bring up my morning erections to him and told him that some days I wake up with one and other days I don't and that i am concerned that this may be a physical erectile problem rather then a pyshcoloigcal one and he told me that it is nothing to worry about and that I should just meditate and try to get rid of my anxiety. I've had sex multiple times after this situation and each time I was able to get an erection naturally although I do not think my erections are as "hard" or rigid now. Also, sometimes when I masturbate to pronography now I am not able to get an erection and I sometimes still do ejaculate prematurely even during masturbation. Is this all due to anxiety? This has been on my mind literally every second of every day for the past 4 months and I am really feeling hopeless that all of doctors are not willing to do an ultrasound to see if maybe one of my penile nerves or muscles are broken. What if I have a vein in my penis that is leaking? Sometimes when I masturbate I lose my erection quickly now (it never happened before). Also, when I do get monring wood I lose it within seconds of waking up whereas before I would have the erection until I got up and went to the bathroom to urinate. If my erection issues were really due to anxiety wouldn't it only be related to performance anxiety? Is the fact that I am thinking about my erections so much causing my brain to not give me regular erections as before? I've had anxiety before with work, school, family ,etc. but never had any erection problems because of it. Do you think this started because of my realization of my phimosis and is no getting worse because I (wrongfully?) disganosed myself with some type of erectile issue? If this really were a psychological issue wouldn't I only have erection issues when I had sex and not during masturbation, morning erections and premature ejaculation? What are the chances of a 26 year old having physical issues with erections vs psychological? I know that the older I get the more my erections will decline but this seems like a HUGE and sudden decline whereas everybody else would go through this process gradually and not noticeably. The only thing I was able to find on the internet that may be effecting my erections would be constant pornagrphy viewing. Is this proven? I do watch pron often and I read online that watching too much pron will cause ED in young men. I am just so sad right now and I can't consentrate at work, I am very easily irritable now and just very annoyed that I can't get erections like I used to. Can you please advise?
