Question: Mam me again bothering u..she didnot get her periods yet..sexual act took place on 22nd July,light period on 25-28th July,then medium period on 15-18th August,then heavy period on 5-9th September,BHCG done 2 months after the act was less than 0.100 miu/cc..usg showed empty uterine cavity on 18th September,2 months since the act..i hope u remembered me now mam..She did USG pelvis abdominal on 9th October..the report states that-

Uterus normal in size shape outline and echotexture..measures 74*37*40mm..Central echo is normal..uterine cavity is empty..cervix is normal..no free fluid in abdomen..ovaries are normal in size..8-9 immature follicle seen at the periphery..RO-27*25*21mm,LO-26*24*22mm..

Impression:Small follicles peripherally arranged in both ovaries-consistent with pco
No GSac seen in uterus/adnexa at present..

Today is 45 th day,still she didnot get her periods..

1)So mam it is related to PCOS only and hormonal imbalance that she didnot get her periods?

2)Mam her gynaecologist is out of town,he asked her to contact him after some days..my question is that if she takes tablets for withdrawal bleed as per her gynaecologist's advice and yet dont get periods after stopping the tablets,what will it indicate?..as i heard that even if withdrawal bleed dont bring periods after 7 days of stopping tablets,it means pregnancy..thats why i am asking..

3)Mam in the report it states that No gsac seen in uterus/adnexa at present,so there is no chance of formation of gsac without any sexual act leading to pregnancy after 22nd July?

4)Mam how is her overall usg report?

Now, regarding your doubts -
1. The delayed period is due to PCOS and hormonal imbalance, as the last sexual act was on 22nd July, and nothing has happened thereafter.
2. SHe will definitely get her periods after taking tablets for withdrawal bleed. It cannot mean pregnancy, since there was no intercourse after 22nd July.
If she does not get a bleed after the tablets, it can indicate severe hormonal imbalance, needing more strong dosage.
3. There is no chance at all of pregnancy without sexual act after 22nd July.
4. Overall USG is fine, suggestive of mild PCOS.

1)Ok mam this is the doubt which i had in my mind..actually i read that if after taking withdrawal tablets periods dont occur it can mean pregnancy thats why i asked u..so even if she dont get periods after stopping withdrawal bleeds it wont indicate any pregnancy with that act on 22nd July?

2)Even if she misses her period for another 15-20 days or more without taking any tablets for withdrawal bleed,still it wont indicate pregnancy with that act?

3)Mam what is central echo?is it the same as ET,midline echo?

4)Mam does uterus size varies?on 18th September it was 65*30*40 mm,on 8th October it was 74*37*40 mm?Is it increased,though it mentioned in usg that its normal in size shape outline and echo texture..

5) Mam does ET also decreases without any external bleeding?on 18th it was 6.7mm,on.8th it was 5.9mm

1)Mam in the report it states no gsac in uterus/adnexa at present on 8th October,so without any sexual act leading to pregnancy after 22nd July there is no chance at all of formation of gsac?

2)No gsac in adnexa and adnexas are free also means ectopic pregnancy is also ruled out?

3)So even if she dont get periods after withdrawal tablets it indicates severe hormonal imbalance only which needs to be rectified with high doses of hormone tablets?

4)Mam u said earlier ET increases with cycle,bt hers ET reduced to 5.9mm without any period or bleeding..

5)Mam does her uterus size of 74*37*40 mm and ET of 6.7mm also corresponds to non pregnant state?

6)So no tests(HPT,BHCG,USG) are further needed to confirm her non pregnant state with that 22nd July act?

1, Yes, as stated earlier, there is no chance AT ALL of formation of g sac without intercourse after 22nd July.
2. Yes, free adnexa means that ectopic pregnancy is completely ruled out.
3. Yes, absence of withdrawal bleed means need for stronger / modified dosage.
4. ET fluctuates, especially in women with PCOS, the difference is only a few mm, maybe individual variation in measurement.
5. Yes , the dimensions of uterus and ET correspond to non pregnant state.
6. NO tests at all are needed to confirm non pregnant state, in fact there are no more tests left in the world any more, all possible tests have been done, and clearly show that she is not pregnant.

1)She got her periods yesterday on the 47th day and its heavy with cramping and heaviness in lower abdomen..is it normal?

2)This time also its heavy flow like September period with cramps..on July 25-28 and August 15-18 it was medium flow,september and october period is heavy..

3)So no chance at all of pregnancy with that 22nd July activity after which she got 2 medium periods and 2 heavy periods?

4)So we should be rest assured about her non pregnant state with that act in July even if her future periods gets preponed,postponed or missed with variations in flow or pattern like spotting or less flow or colour change?

5)Why is the heaviness in lower abdomen and cramps now?

Mam she got this time period from 21st October to 26th October morning..

1)In July and August,the flow was light to start with with on 1st day,then went medium on 2nd and 3 rd day morning, then went light,finally went away on 4th day,whereas her period for September and October was heavy to start with for 1st 2 days,then normal on 3 rd day and then went light for 4th and 5 th day and finally went away on 6th day morning..why is this difference in no f days and amount?because of PCOD?

2)The lower abdomen heaviness and cramps were for 1st 2 days of period,then went away..is it normal?

3)Mam as u said that in pregnancy uterus gets bulky and enlarged,so if her uterus gets enlarged and bulky without any sexual act after 22nd July,what will it indicate?

4)Mam does uterus size varies through out the cycle?..Her uterus size on 15th September was 7.1*2.6*3.7 mm,on 5th October 59*30*40 mm and on 7th October its 74*37*40 mm..is it increased?though in the report its mentioned its normal in suze shape?

5)In pregnancy ET increases,but if her ET increases without any sexual act after July,what will it indicate?

1. In a normal woman, menstrual flow pattern and duration varies, it is not exactly the same every month, and yes, because of PCOD, the pattern is even more irregular and unpredictable.
2. Yes, it is quite normal for cramps to disappear after 1st 1 - 2 days of periods.
3. If the uterus gets enlarged and bulky without any sexual act after 22nd July, it might be due to PCOD, hormonal imbalance etc
4. THe uterus size does not vary much, observer variation might be there.
5. Increased ET indicates endometrial hyperplasia ( hormonal condition ) commonly seen in PCOS.

2)So even if uterus enlarges now it has nothing to do with that act?

3) Does uterus size varies along the menstrual cycle?on 14th day it was 65*30*40 mm and on 33 rd day it was 74*37*40 mm..so it varies along the menstrual cycle and nothing abnormal in the size?

4)So even if she misses her period in future it is due to PCOD and hormonal imbalance and not due to the act?

5)Even if she misses her future period even after taking withdrawal tablets,it needs to be rectified with higher dosage and not at all related to the act?

6)So even if her uterus increases and gets bulky and enlarged it cannot be at all related to pregnancy with that act in July?

7)Is her uterus size of 74*37*40 mm normal?is it increased from previous measurement of 65*30*40mm?

8)So even if her ET gets thickened in future it cannot be related at all to pregnancy with that act in July?

2. Yes, even if uterus enlarges now, it has nothing to do with that act. If she had conceived from that act, she would be more than 3 months pregnant now, and it would show ! Please stop thinking about it now atleast !
3. Yes, it might vary a bit , but there is nothing abnormal in the size.
4. Yes, missing the period is due to PCOD and hormonal disturbance.
NOTHING that happens to her can be due to an act that happened 4 months ago.
5. Yes, it needs to be rectified with higher / different dosages.
6. It cannot be related to pregnancy.
7. Both sizes are normal, the increase is nothing to worry about or think about.
8. NO , thickening of ET cannot be related to pregnancy at all.

1)U said that normal size of uterus is 70*50*30 mm,bt her uterus size was 65*30*40 mm and 74*37*40 mm(though in both the reports its stated that its normal in size and shape)..

2)Mam last bhcg was done on 12 th September(52 days after the act) and it showed less than 0.100 miu/cc,so that value didnot and wont increase later with that act in July?

3) So no chance at all of pregnancy with that act after 2 medium periods and 2 heavy periods?

1. Different organs measure differently in different people ! A difference of 1 cm is hardly a difference at all ! Somebody's heart might weight a couple of grams more than somebody else's , that does not make anybody abnormal.
So if somebody says 80 by 60 by 40, that is also normal ( normal is a range ) and her reports show a normal size, and I think I have made it VERY clear that it does NOT indicate pregnancy in any manner.
2. That value will not increase without pregnancy .
3. No chance at all of pregnancy.
4. No tests of any kind are needed to confirm non pregnant state, whatever her bleeding pattern might be.

1) So that value of less than 0.100 miu/cc wl not increase later after 12 th September, will not increase with that 22nd July act?

2)Mam though its 65*30*40 mm an 74*37*40mm,its written that its normal in size shape and echo pattern in both the reports though they were from different clinics..so everything is completely normal?
