Question: Dear Dr , I have been running a recurring fever for about 10 days. It usually comes back midday and evenings. I definitely got a cold 10 days ago because I was traveling on airplane as well. However this week, all seemed better but I still needed two aspirin a day to keep me afloat. I had cold sweats in the morning up until two days ago when things went awry. I started to feel ill as if this was flu/cold, nothing different in the feeling, however it has been accompannied by urinary tract pain, which first occured 5 weeks ago but lasted for just a week (i had gotten better via baking soda and cranberry extract pills!

However Friday evening I fell into bed very tired and had no sweating overnight, my fever hitting around 38 celsius, and only after an aspirin it got better about a 0.7 of a degree. Today however I woke up the same or even a little worse, but since yesterday I have also been feeling bladder pain so I took an antibiotic. It had gotten so much better over the afternoon today however I now have the chills and 38.4celsius fever again. I took 500mg of amoxicilin.
What do you suppose I should do? I am a bit stressed out by now.
I have no loss in taste or no runny nose or heavy throat!

Hi,

Fever usually indicated an infection which could be viral or bacterial.. However, viral infections such as influenza virus as usually self limited and the fever dose not last more than three days.. This is why the infection that has caused your fever is probably a bacterial one.. And since you are experiencing bladder pain then this is probably a urinary tract infection.

The recommended treatment protocol should have been based on a urine analysis accompanied by a culture and sensitivity test to detect which type of antibiotics your infection is most sensitive to.. However, since you have already started amoxicillin then it is better to continue as discontinuing an antibiotic without completing its course may result in a more resistant infection.

So, please take amoxicillin 500 mg every eight hours for 14 days. Also, you should keep yourself adequately hydrated with at least eight cups of water daily to help clear the infection.

The fever is problematic though because it jumps every time I eat something (just ate a very light lunch, but have no problems with taste), however with an ibuprofene-like drug, it came down a bit (about a degree). The only real relief I have felt during the last few days has been with antibiotic use today, because the fever fell below 37 degrees celsius and I also didn't have a headache. Have been drinking about 3 l of cold fluids, so that helps a bit.

I have a few questions now:

- first of all, why is the fever returning?

- if the fever persists or at least keeps returning, do I smack it down with aspirin or something else or just drink up or anything else? I usually drink lemon water with ginger and now I also drink some immune boosting tea

- can I take any vitamins during this period?

The fever is returning because the infection has not cleared up completely. To reduce you body temperature, I would recommend ibuprofen tablet 200 mg every 6-8 hours. If the fever keeps recurring while one ibuprofen, then you should use aspirin in alternation with ibuprofen every 4 hours. Yes, you can take a multi-vitamin supplement while on the antibiotic and antipyretics.

Dear Sir,

just one more question: I have had in general problems with my sinuses and in the recent two days my biggest problem is a pounding headache in front of the head, which only got a little better when I inhaled some tea today. It seems to me my sinuses are clogged as well. Could this be the true cause of fever/pounding headache?

Because my bladder problems seem to be ok, I have ok color of pee, no pain while urinating, absolutely no bad color and no pain when I press the bladder.

Hello,

Recurring fever could be due to an infection anywhere in your body. However, you should have had a urinalysis before starting the antibiotic to confirm the diagnosis. It is not beneficial to get a urinalysis after starting the antibiotic as it will lead to a false negative result. Anyway, the antibiotic you are using will help clear any sinus infection and you can add an antihistaminic as Cetirizine once daily before going to bed to clear any congestion. for a headache you can take ibuprofen tablets 200 mg when needed.
