Can TMT Show Mildly Positive For Inducible Myocardial Ischemia For A Healthy Person?

Question: Can TMT Show Mildly Positive for Inducible Myocardial Ischemia for a healthy person some times. My consulted doctor told me that this result for me in TMT was due to other reasons and not related to Cardio. What are the other reasons for Mildly positive result.

Hello, I am Dr Mody and I would be addressing your concern
Yes, I would say in 25% cases it can be false positive. It means that even if it's positive, one will have no heart blockage's if that's what you want to know.
Many a times if your hemoglobin was low, blood pressure on higher side than you may have a mildly positive stress test.

Pre test probability, ie. If you have risk factors like you are obese, bad lifestyle, chronic smoker, kidney issues, diabetic, hypertensive, have symptoms of breathlessness or chest discomfort while you walk or exert than further evaluation is needed otherwise nothing required.
Do upload the TMT so I can interpret it for you.

Hello, I did go through the TMT reading. It is very much normal and the changes noted are due to the baseline ecg.

As I said if you are symptom free and no risk factors, just forget about it. Repeat a TMT in any hospital after 6 months and this time walk XXXXXXX till 100% (target heart rate), this time they terminated at 88% ( most labs do so in diagnostic center settings) to know any change in the treadmill stress test results by carefully comparing the ecg's.

Mostly your 2d echocardiography would have been done and it should be normal at baseline.

Make sure your lipids are well under control, you are in good life balance with healthy food habits and adequate exercise everyday.

Do update me after 6 months about the TMT.

Sir,

Thank you very much for your prompt response.

1) My 2D Echo Cardio Graphy Result was Normal.
2) My Cholosterol was 197, Creatine 1.0, Urea - 37 and remaining were well within limits.
3) I have high BP and taking Telma H and for Cholosterol I am taking Atorva 10 mg.

Sir, please tell me on good life balance with healthy food habits and adequate exercise every day. Will 30 mins brisk walk do. I have already reduced intake of oil and salt.

The reports mentioned are pretty much in normal range
Telma H is a good medication for hypertension
Yes you may take Non Vegetarian Food
Some important things you should follow are.
1) try maintaining blood pressure upper under 135 and lower under 85mm hg with lifestyle and medication
2) if overweight, losing weight should be a priority, try achieving BMI less than 25.
3) 40-45 mins of aerobic exercise daily
4) Be regular with your medication, don't skip doses or stop abruptly.
5) in diet
-low salt. XXXXXXX 3 gms a day.
- avoid preserved food as all preservatives have salt.
- no pickles, papads, chips, Farsan as all have high salt and saturated oil which is not good for you .
- atleast 2 serving of fresh fruits in a day.
- 1 serving 35-40 gms of nuts like walnuts, almond to be included
- if you take fish 2 serving of fatty fish per will like salmon.
-avoid red meat, egg yellow, fried and spicy oily food
- may shift to better oils like olive in cooking
6) may consider supplements, like cod fish oil capsules, flax seeds, Chia seeds, krill oil as they have good content of omega 3 fatty acid.
Every 3 yearly treadmill stress test for heart, urine microablbumin and check up of your eyes for retinopathy for any Ill effect of hypertension on your body.
As you are young our aim is not just to control blood pressure but protect end organs from any further damage.
