Can BP Medicines Cause Gout Attacks?

Question: My husband is taking several meds for blood pressure and heart. Are there any that could cause gout attacks?
the meds my husband take are many.
Plavix 75 mg.
amlodipine Besylate 10mg.
Colcrys 0.6 mg.
Isosorbide Mononitrate 30mg
Metoprolol Tartrate 25mg. 1/2 tablet Twice daily
Simvastatin 40 mg.
Mapap Caps(I think this is aspirin)650 mg.
Vitamin D 1`000 unit caps.
SODIUM BICARB)
Diovan 320 mg. Triamterene -hctz 37.R -25 MG. RECENTLY Changed to Valsartan 320-25 Ins. would no longer pay for Diovan
Uloric 40 mg.(febuxostat
Oxycodone HCL 5mg. up to 4 times a day. He also takes a stool softener when he takes the oxycodone He trys to stay away from this med.

He also takes prednisone 10mg. when the pain of gout gets more than he can stand. One part of his body will clear up and than pain will pop up in another place . Today the pain is in his right elbow and wrist . We have been trying to watch what we eat but it seems as if everything is setting it off. Could you supply a list of things we should avoid.

Diovan is the brand name for Valsartan. I know it is an expensive brand. Valsartan 320-25 can also do the same work perfectly. Can you please give the dosage of Valsartan you are taking? Does it contain hydrochlorothiazide too? If yes, should discuss with his doctor about triggering of gout attacks by hydrochlorothiazide.

I have noticed that your husband is taking 2 drugs for gout attacks:
- Colcrys
- Uloric

If you try to control gout attacks (flare ups), his blood pressure will be also optimized.

It is good that he is taking Mapap Cap as it will dominate inflammation. To my records, Mapap Caps is the brand name for the painkiller acetaminophen.

It is normal for gout attack to change location (elbow, wrist, or any other place).

The very first thing I recommend to my patients with gout is to drink plenty of water. Thus, he will be well hydrated preventing attack to occur.

Next, as his doctor might have advised, he should :
- not drink alcohol
- eat meat (especially red meat). He should limit its intake as per his health condition
- if he is overweight, should start losing weight
- physical activity is also very important
- avoid having drinks high in fructose

All these preventive measures have helped many of my patients improving the symptoms and preventing attack onset.

You say avoid having drinks with fructose. What is fructose and what drinks contain it? Valsartan dose is 320-25 mg.. Yes I believe the valsartan contains the hydrochiorothiazide because the script bottle has the letters HCTZ. He takes 1 caplet in the morning. Do you know if caffiene could be a gout inducer. Many people say orange juice. My husband used to drink oj for breakfast now he only has milk. If we eat out he will drink un-sweetened ice tea. Any information you could give me is greatly appreciated.

Fructose is a type of simple sugar, commonly found in fruits. Fruits are good to eat. The bad thing is when fructose is served as sweetener in soft drinks, especially when it comes to corn syrup fructose. These types of foods/drinks are to avoid.

Next, if the script bottle has the letters "HCTZ", then valsartan you are taking has hydrochlorothiazide. You can discuss with your doctor about the potential triggering effect of HCTZ to gout attacks and look for other diuretic alternatives. Please bare in mind that HCTZ is a diuretic used to treat hypertension.

With regards to caffeine, yes, you are right, caffeinated drinks/foods, if taken in high doses, can induce gout attack. If he loves coffee, can try decaffeinated and see if that can help.

Orange juice is a fruit juice. As mentioned earlier, fruit juices do contain fructose. Therefore, their regular intake might trigger gout attack.

I usually recommend my patients to look for foods that won't trigger attacks. Although there is a list provided, however, it will all depend on how his body will react to that particular food/drink and if gout is reactivated. If yes, then, should avoid that food/drink for life.
