What Does Normal Spleen With Hypoechoic Mottling Of Parenchyma Indicate In Ultrasound Report?
hi my name is Zan. please help me. can you tell me what the following means?
Ultrasound findings: the spleen is normal in size and has a mild uniform hypoechoic mottling of entire parenchyma.both kidneys appear to be reasonably normal. there is no abvious free abdominal fluid. the adrenal glands were not seen. the ovaries were not seen. the bladder is small and empty and has a prominet irregular mucosal surface. there is a small amount of mildy speckled urine. the caudal half abdomen contains a thin walled cystic structure measuring greater than 15 cm containing hyperechoic fluid. within the lumen there is a 10 cm irregular hyperechoic mass consisting of multiple irregular cysticstructures, and which is attached to the dorsal surface of the cystic structure. it cannot be ascertained wheather this mass is uterus or ovarian tissue. the medial iliac lymph nodes are enlarged and prominent measuring up to 2 x 3.5 cm and contains multiple 5mm hypoechoic nodules. the liver appears largely remarkable. the gall bladderis normal. theres no obvious pancreatic pathology. gastrointestinal tract appears unreamarkable.
ultrasound diagnosis: abdominal mass, originating possibly from the uterus or ovary with evidence of metastatic spread to the medial iliac lymph nodes.
please please explain to me what this means?!

1.The report suggest there is altered echo pattern in the spleen parenchyma suggest there is some abnormal blood picture which need to evaluate. 2.There is a mass lesion in the pelvis with cystic in nature - the doctor suspicion goes towards the ovarian or uterine pathology / lesion. 3.It was not able to delineate the exact extent or origin due to technical limitation of the ultrasound. 4.There are multiple enlarged lymph nodes seen suggest some active infective / inflammatory / malignancy can be suspected. 5.In this case MRI with contrast will be ideal to evaluate the underlying cause and well delineation of the anatomy.
